Hello doctor,

My father has been suffering from mild fever for 12 days ranging from 96.5 to 99.2. We have done his urine test. He is 69 years old and has a long history of prostate enlargement. What should we do? He is on Afdura and Taxim O for the past two days.

Thanks for adding the blood and urine results (attachment removed to protect patient identity). Your father is suffering from prostate enlargement complicated by urinary infection and fever. It is actually worrying sign as it might develop and progress to kidney failure and sepsis (bacteria in the blood) that is why you need to decompress the bladder first by inserting a urinary catheter, so you can drain the urine. In addition, all the mentioned antibiotics are injectable so he needs admission for and check on his kidney functions. Then repeat urine culture after 48 hours of starting the injectable antibiotics. Some times in cases like this, patients deteriorate quickly and it is not wise to monitor the patient like this at home. He will ultimately need to clear the infection. If kidney functions were high and got normal with catheter then medical treatment is not indicated and have to proceed for endoscopic operation. But if his kidney function is normal then medications can be continued and upon removal of catheter he needs uroflowmetry and bladder scan to check the pressure of flow and residual urine then decide whether he is safe without catheter or he still needs an operation to remove the obstructing prostate.
