Hi doctor,

Initially, I was having right abdomen pain about two to three weeks ago when I visited the doctor, who then performed blood, urine test, ultrasound, computed tomography (CT scan), and another ultrasound and confirmed all my organs are fine. I was born without a gall bladder. I have all the results. However, now the pain is back again. I am currently having abdomen pain on both sides of my abdomen just below the rib cage and experiencing back pain on both sides.

From the reports (attachments removed to protect the patient's identity) and your history, yes, the gall bladder is absent, and the pain your experiencing has to be evaluated with a further investigation like

1. Endoscopy.

2. Contrast scan of the abdomen.

Everything in the reports looks quite normal.

Just continue a proton pump inhibitor and eat small and healthy meals with complete avoiding of alcohol and smoking.

Tablet Pantodac DSR (Pantoprazole and Domperidone) (1-0-1) for one week before food is suggested.

Yes, I did check. You have a urine infection.

UTI (urinary tract infection) pain can even radiate and cause back pain, but I asked you to rule out those investigations for abdomen pain. For urine infection, what your taking is sufficient, and do continue the complete course with that drink plenty of fluids.

Hi doctor,

Can one use a combination of Gentamicin and Ciprofloxacin (If so, in what quantities and doses as in 500 milligrams) to treat Serratia marcescens bacteria. Is it possible that these bacteria can result in kidney pain? What test should I perform to check my abdomen after I finish my course to ensure the bacteria is gone? As to confirm that my kidney and other organs are fine. How difficult is it to treat Serratia marcescens?

I am a bit worried that this could cause permanent damage to my kidney and other organs or end up developing pneumonia.

Yes, a combination of both can be used with the dosage of Ciprofloxacin 500 milligrams in tablet and Gentamycin in injection form of 80 milligrams, but usually, we prescribe Ciplox TZ, which even has Tinidazole. Start with Ciprofloxacin 500 milligrams only in morning and night. After the treatment is done, the urine should show no growth and just get a urine routine to see pus cells, epithelial cells, and red blood cells. And regarding pneumonia, you should have symptoms of chronic cough with sputum and lung-related issues. Get a culture sensitivity test and routine after your done with the treatment. And add on to note you can check your report it shows briefly whether your bacteria are sensitive to and susceptible so in that case you are on good treatment.

It can cause respiratory system, gastrointestinal tract, and urinary tract infection. Kidney pain can be a complication of urinary tract infection (UTI). Drink plenty of fluids not only till treatment but even after that.

Hi doctor,

I further went to the doctor two days ago and they did another test as my condition is not improving if you look at my record above the pain has spread from the lower right abdomen to the right and mid of the abdomen. And it becomes more painful when I sit down. Please see the results of the test attached. I was given Cefuroxime which seems not to improve my situation. Kindly check from the results is my kidney, liver and other organs all are healthy.

Can you get on a call with me. I will explain you everything. You can talk there. The lower right abdomen to right and mid of the abdominal pain you are experiencing is due to helicobacter pylori infection. H. pylori is detected, and you need to start treatment for that. In India we give the kit a set ofpil O (combination of ornidazole and ofloxacin). These tablets are taken in morning, afternoon, and night. It comes in the kit. I am unknown of the guidelines of treatment in your country. Once your treatment is completed you will have relief and you should take another serology test for the negative report. The other aspect which I want to bring to your notice is it is typhi and paratyphoid positive. How many days it has been you have started with Cefuroxime and was the test conducted during the treatment?

How do I call you? Anyway, the medication I started it on Wednesday, but I still feel the same pain. I was vaccinated for typhoid about four years ago. The test was conducted before I got the Cefuroxime. I was on Ciprofloxacin 500 milligrams before and after completing the treatment. I went to the doctor and got tested and the result is the one that you have just looked at.

Get Typhi and paratyphoid test done now to see the result. It should show non-reactive. You need to get treatment for typhi and Para typhi. Get ultrasound abdomen and pelvis done before starting the treatment. You need complete evaluation on helicobacter pylori infection. Now start with tablet Pantodac DSR daily before food morning and night, twice a day.
