Hello doctor,

I got vaginal ulcers after suffering from flu-like symptoms for a few days. I was diagnosed with human papillomavirus (HPV) earlier. What should I do?

I read your query and can understand your concern.

As you had a history of flu, these ulcers mostly look like Lipschutz ulcer, associated with influenza or flu virus infection. They appear as bilateral, symmetrical (kissing) vulvar ulcers on the labia minora. The ulcers are 2 cm in diameter and have a necrotic appearance and a fibrinous base, supporting, painful, and foul-smelling.

The other possible causes include cytomegalovirus, Epstein-Barr virus, and bacterial infections.

It is essential to do some investigations to know the accurate diagnosis.

  1. Serology tests for HIV (human immunodeficiency virus) 1 and 2, HBsAg (hepatitis B surface antigen), HCV (hepatitis C virus), toxoplasma, herpes simplex virus types 1 and 2, parvovirus, cytomegalovirus, Epstein-Barr virus, parainfluenza virus types 1, 2, and 3, adenovirus, and influenza B virus.
  2. High vaginal swab culture and sensitivity.
  3. Vaginal swab for sexually transmitted infections.
  4. PCR (polymerase chain reaction).
  5. PAP (Papanicolaou) smear.

Also, it would be best if you visited a local doctor for a per speculum examination to know the extent of the spread of this infection and avoid any sexual contact till the ulcers are healed.

I also suggest the following medications.

  1. Tablet Amoxicillin and Clavulanic acid 500 mg every eight hours for seven days.
  2. Tablet Ibuprofen 200 mg twice a day after food.
  3. Some topical vaginal dressings or ice packs for the vagina.

However, please consult a specialist doctor, discuss with them and start taking the medications with their consent.

What kinds of topical creams do you recommend? Does this cause the discharge as well?

I suggest you apply Hydrogel cream topically and let it air dry twice a week. Please consult a specialist doctor, discuss with them and start taking the medications with their consent.

Yes, the discharge is from the ulcers. However, if it is not treated on time, it will lead to secondary infections.
