Hi doctor,

I am a 21-year-old female and I have not been able to digest any food. As soon as, I eat anything or drink water, I vomit within a few hours. I feel tired all day. Last week, I had a fever and then I got my periods, and this has been continuing since then. Also, my smell and taste has also become worse.

I have gone through your complaints.

The history of fever, vomiting and weakness suggests that you may be suffering from some infection. It may be typhoid, leptospirosis, or scrub typhus.

You should get NS1 antigen test (to rule out dengue), HRP-II and pvLDH test (to rule out malaria), peripheral blood smear for malaria and CBC (complete blood count) done.

The Probable causes

The probable causes include typhoid, leptosiprosis, scrub typhus, malaria and dengue.

Investigations to be done

Investigations include complete blood count, peripheral blood smear, HRP-II and pvLDH, NS-1 antigen.

Differential diagnosis

The differential diagnosis include leptospirosis, scrub typhus, malaria and dengue.

Probable diagnosis

The probable diagnosis include typhoid.

Treatment plan

The treatment plan includes, 1. Keep proper hydration with plain water, fresh fruit juice or so. 2. Take a tablet of Paracetamol 650 mg, one tablet in the morning and one in the evening with food for three days. 3. Tablet Ondansetron 4mg, one tablet in the morning and one tablet in the evening with or without food for two days.

Regarding follow up

Followup for with blood reports.
