I have a painful vulva that could be due to scratching too hard. I also have a wart, and not sure if it is HPV. It does not look like it. I am using Warticon cream.

I have reviewed your attached images (attachments removed to protect the patient's identity).It seems like a chronic infection of the vulva. It is mostly due to fungal infection. Continuous scratching has made skin blackish due to over-irritating the same place. Another lesion seems like a follicular infection means hair follicle infection when you are peeling off. You can use antihistamines like Levocetirizine or Fexofenadine for itching. Use antifungals like Itraconazole for oral use or Luliconazole cream for local use. You can Metronidazole for follicular infection. Use local anesthetic cream for a soothing effect and a less irritating feel. Consult your specialist, doctor, and take medicine with the doctors consent.

I cannot differentiate it from a wart as it seems bacterial infection too. So if it is a wart then it is due to common wart infection condyloma but if it is a bacterial infection then it must be evaluated to get a proper diagnosis.

The probable cause is a bacterial infection and fungal infection.

Investigations to be done are

  1. CBC (complete blood count).
  2. Random blood sugar.
  3. Vaginal swab culture and sensitivity.

The differential diagnosis is warts.

The probable diagnosis is follicular Infection. The treatment plan includes antibiotics, antifungals, and steroids for local usage.

Preventive measures are avoiding wet clothes and using cotton clothes. Always try to maintain genital hygiene.
