Patient's Query

Hi doctor,

I am a 28-year-old male. My height is 5 feet 9 inches and weight is 176 lbs. I am HIV (Human Immunodeficiency Virus) infected and I got red blisters on my tongue with a white coating on the tongue surface.

Hello,

Welcome to icliniq.com.

Your complaints are more in favor of oral candidiasis. This is an opportunistic infection in the immunosuppressed patients. Kindly mention your CD4 count, RNA (ribonucleic acid) load, and the medication that you are using. Please attach the appropriate images.

Patient's Query

Thank you doctor,

I have no report to show you and I am not on ART (Antiretroviral Therapy) medicines at all. Please suggest medications if possible.

Hi,

Welcome back to icliniq.com.

Images of your tongue and oral cavity would be really helpful to confirm the diagnosis before starting any medication. The treatment of choice for you would be Fluconazole, initially in a loading dose of 200 mg, followed by 100 mg daily for two weeks. Consult your specialist doctor, discuss with him or her and start taking the medicines with their consent. Candid mouth paint local application twice daily, after food and mouth gargling will help to attain quicker relief. Please upload appropriate images.

Patient's Query

Thank you doctor,

Currently, I was taking these medications because of pain in the throat. In the first and second month of this problem, someone prescribed Fluconazole 150 mg for two weeks with B complex and then someone took it to be tonsils and gave also Fluconazole with some other medicine. But till now, I have no benefit from all these. I am sending you an image and the last prescription. I also have a dry mouth problem with this and have to drink a lot of water. Please guide me the best.

Hello, Welcome back to icliniq.com.

From your image (attachment removed to protect patient identity), definitely, it appears that your tongue has a white coat. As you are known retropositive, oral candidiasis would be the first possibility. The medication prescribed by your doctor was meant for your nose and throat infection. At times, Alaspan (Loratadine) tablet (taken by you), can cause dryness of mouth. In your case, I would suggest you to take Fluconazole 200 mg on the first day, followed by 100 mg daily for two weeks, along with applying candid mouth paint twice daily after mouth gargling.

Same symptoms don't mean you have the same problem. Consult a doctor now!
