Hello doctor,

I am a 35-year-old female, and I am having painful lower abdominal cramps for three days, which got relieved by taking muscle relaxant medication. My last menstrual date was 21 days ago. I have a history of ovarian fibroma and cysts.

Please, suggest the reason for these cramps.

I read your query and understood your concern.

Based on the information you provided, I understand you had undergone surgery for ovarian fibroma and cysts. Usually, after the healing of the cyst or after surgery, adhesions develop around the uterus. However, this could also be due to any infections ascending through the cervix which occurs most commonly through intercourse. During the post-luteal phase which is 14 days after ovulation, blood flow to the uterus significantly increases. The uterus is surrounded by muscles and ligaments. Hence, when the blood flow increases, all these attachments becomes tense. This may cause pain in the lower abdomen, radiating to the back, thighs, or vaginal areas. Once the periods are over, the pain also reduces.

If you are facing issues with pregnancy, I suggest you consult a specialist and get tubal patency checked, as these adhesions may also block the tubes which stop egg fertilization. I suggest you take a painkiller for pain relief. But avoid this medication if you are trying to get pregnant. As these medicines may interfere with ovulation and cause the fertility issue.
