Hi doctor,

What kind of birth control is necessary to help clear up my hormonal adult cystic acne that has lined my entire jawline from ear to chin with some dark scarring? I have heard there is a specific BC that two chemicals are necessary for this to help. I have been using Cetaphil cleanser, witch hazel afterward, with a light-based moisturizer. My spots flare up and down daily. Please help.

I understand your concern.

I examined your picture (the attachments are removed to protect the patient's identity) with your descriptions. Your problem seems to be acne vulgaris. Acne vulgaris is a common skin concern in adolescents and may sometimes persist in adulthood too.

  • It is a skin disorder involving pilosebaceous follicles or oil glands and is seen to involve areas where oil glands are dense. The common sites of involvement are the face, chest, back, and upper arms.
  • In females, it may also be associated with menses irregularities or PCOS (polycystic ovarian syndrome).
  • You must have specific dietary restrictions like not taking oily and spicy food, junk food, milk products, and non-vegetarian.
  • Drink lots of water that hydrates your skin, and avoid applying hair oil because it can aggravate your forehead acne.
  • You can use Cutiwash face wash (aloe vera and Glycolic acid) or alcohol and acetone-free cleanser or soft soap twice daily.
  • I suggest the tablet Azithromycin 500 mg once daily for three consecutive days a week for four weeks. Also, a capsule of Isotretinoin 20 mg once daily for six weeks.
  • It is helpful to use topical antibiotics cream like 1 % Clindamycin gel over the pimples in the daytime over the face.

Before starting the oral medication, it is best to get the following tests done. Liver function tests and lipid profile. If these come standard, you can start with capsule Isotretinoin after consulting your doctor or nearby dermatologist. Still, you must know Isotretinoin is a teratogenic drug that must be prescribed by a dermatologist only. There should be no pregnancy or conception during and even three to six months after stopping the medication.
