Hi doctor,

I hope you are well. I am 25 years old, and I suffer from severe acne, especially on my cheeks and jawline. I have very oily skin, maybe that is the reason. Can you help me build a face routine that will rid me of acne and help me get rid of the acne scars? I really hope that you can help me, my self-esteem has been affected by my acne problems. I have at home the following items that may be useful: A facial steam machine, sulfur ointment, peeling solution, acne facial wash, toner, salicylic acid touch pads, aloe vera gel, tea tree oil.

Hi doctor,

Thank you for your reply. I have attached photos for your reference, please take a look and help me find a cause and a treatment for my condition. This has been reoccurring for the past year, getting more severe recently, but never fully subsiding. I suspect hormonal factors, but my period is quite regular (three days long, every month on time), and no excess hair growth. Therefore, I ruled out the possibility of elevated apagen. So I do not have a clue what the cause may be. Could it be sensitivity to a particular food? A bad diet, stress, or just having oily skin?

Thanks for sharing the pictures (attachment removed to protect patient identity). You have acne vulgaris, there can be a hormonal component also, but you do not have any history suggestive of that as mentioned in your message. Acne vulgaris has multifactorial etiology including bacterial infection, increased sebum (oil) production and increased sensitivity to androgen hormone along with certain dietary factors. Start following medications and get some investigations done as mentioned below:

1. Tablet Azithromycin 500 mg three days a week like every Monday, Tuesday, and Wednesday.

2. Adapalene plus Benzoyl Peroxide (Epiduo) gel apply once at night on acne in little amount and wash after one hour, do not rub it.

3. Benzoyl Peroxide face wash.

Avoid any other face product except may be a moisturizer as the topical gel may lead to a bit of dryness.

Investigations:

1. LFT.

2. Fasting lipid profile.

Get these investigations done. If normal I would like to start another oral medication along with the prescribed above, named Isotretinoin. Avoid foods with a high glycemic index like full cream milk, chocolates, and sweets. Take care.
