Hi doctor,
I am a 47-year-old male suffering from an acute sore throat, swelling, and ear heaviness for three weeks. In the first week, I took Cefixime for six days. Then, I took Cefuroxime 500 mg along with Winolap (Olopatadine) and Dolonex (Piroxicam) for seven days.
This week, I am experiencing a sore throat, tonsil pain, swelling in the throat, stiffness in the throat walls, and postnasal drip. I have been taking Cepodem XP and Montair for three days, but I still have thick mucus at the back of my throat with post-nasal drip. Even a full-swing fan exacerbates my sinus problem. My uric acid level is 6.81 mg/dL and my HbA1c is 6.3 percent. I am not diabetic or hypertensive.

Please indicate whether the throat pain is severe or tolerable. Also, let me know if you have a fever, muscle pain, or weakness along with it. Please mention whether you feel better after taking a different course of antibiotics or if you feel the same.
Do you have nasal blockage due to an air conditioner or ceiling fan?
Please indicate if you have a runny nose or sneezing. Please send a picture of the back of your throat with your mouth open, showing the affected area. Please mention if you smoke or drink.

Thank you for the reply.
Initially, the pain was severe, but it reduced after doing steam inhalation and taking Dolonex. Now, I have mild throat pain, along with pain in my ankles, legs, and knees, and slight weakness. I feel much better after taking Cepodem XP and Montair FX. However, I still have redness with post-nasal drip and white pus sticking to my throat and tonsils. The air conditioner and full-swing fan make my ears feel heavy. Sometimes, I experience sneezing with a runny nose. I do not smoke or drink alcohol.

Your throat pictures are clear (attachment removed to protect patient identity). You do not have any infection currently. You are having some reflux associated with laryngopharyngitis and rhinitis. This occurs commonly after a course of acute pharyngitis. This needs medicine to suppress allergy, postnasal drip, and reflux. This needs dietary and lifestyle changes. You can take a tablet Raciper 40 (Esomeprazole) one tablet before breakfast for two weeks. You can take a tablet Telekast (Montelukast) one tablet in the evening at 7:00 PM daily for two weeks.
You can use Duonase nasal spray (Fluticasone propionate with Azelastine) two puffs in each nostril twice daily after meals for two weeks. You can have steam inhalation multiple times daily. Do not do throat gargling; it is not required for you. Do not skip meals and take frequent meals of lesser quantities. Avoid taking spicy and oily foods. Avoid smoke, dust, and pollution. Do not drink cold water or carbonated drinks. Do not take citrus fruits or curd. Reduce your intake of milk, coffee, tea, and chocolates. Do not drink lots of water immediately after meals. Do not lie down immediately after taking meals. Follow these measures for 10 to 15 days to have some improvement.
