Hi doctor,
My nephew, who is 5.6 years old, has got a skin infection in the right midfoot. My sister noticed it a week ago. She thought it as some infection due to sweat in summer. Now, it has been the same which is in the attached photo. Until now there is no itching or burning or any irritation or pain. There is no swelling in that leg. There is no discharge, and the skin is not peeling over there. It looks shiny and somewhat scaly in dark pink color. It is not puffy, and there is no pitting on pressure. He is not aware of it unless we ask him about the skin change. Please explain what it is and advice the treatment.

I have read your concern and examined your picture (attachment removed to protect patient identity). It seems that your nephew developed fixed drug eruption or irritant contact dermatitis. I suggest you apply Fucibet cream (Betamethasone and Fusidic acid) twice daily and syrup Cetirizine 5 mL once daily for five days after consulting your GP or near by dermatologist.

Thank you for the reply.
We do not feel it as fixed drug eruption as he was not exposed to any new drug in the last 15 days. To my understanding, allergic dermatitis will cause itching right. But, he did not file a complaint of itching or irritation.

Fine, so FDE is ruled out. It is not necessarily ICD always cause itching like in his case. It seems to be blister beetle dermatitis, in such case, mild burning sensation may be there and not necessarily always be there. In my opinion, these two possibilities are there. The best is that in both dermatitis and FDE, the treatment part is always the same.
