Hi doctor,

I have had full body twitches for one month now along with muscle spasms that sometimes wake me up at night or during nap. The twitches are non-stop. I am concerned about ALS. I have muscle tightness in my left thigh as well. Recently I had two large spasms in that same thigh, which were enough to wake me up. All my blood tests are normal. I do not have any vitamin deficiencies. The MRI of the brain and neck was normal. I take Cyclobenzaprine 5 mg twice a day and Buspirone 5 mg twice a day daily. Please advise.

I understand how you must feel with those twitches, cramps, and the constant fear of ALS (Amyotrophic lateral sclerosis). It sounds hard and I am so sorry you are experiencing it. However, the good thing is that I did not find any description of weakness or muscle atrophy in your history. It is good, because those are the main signs of ALS, especially if they occur along with muscle twitches. As you described, the laboratory and MRI studies all were normal. Nevertheless, you did not mention if you had undergone an 'electromyography and nerve conduction study (EMG-NCS)' or not. The result of EMG-NCS is crucial to determine the nature of your problem. Moreover, If I were your physician, I also would prescribe an anti-GAD antibody test for you. So as a summary please inform me if:

1. You have muscle atrophy or weakness

2. The result of EMG-NCS (if you have done).

3. You have done a blood sample for Anti-Gad antibody.

Hi doctor,

I do not have any weakness. Is this something that can come later with ALS? What does anti-had antibody show? Is it diabetes? I have not had an EMG-NCS. Should the medicines that I take not resolve the muscle spasms? Because if so it has not helped. Also what suspicions could these twitches and spasms be attributed to? It is going on for more than 30 days. Please help.

Your question is hard to answer. Fortunately, the most common cause of generalized muscle twitches is 'benign fasciculation syndrome (BFS)'. BFS is annoying, but is not life-threatening, and usually subsides after a while. The main difference between ALS and BFS, clinically, is that BFS does not cause atrophy or weakness. So, based on your descriptions, at least clinically, your symptoms are compatible with BFS at present.

However, EMG-NCS (electromyography and nerve conduction study) is a para-clinic test that can differentiate ALS from BFS. As you have not done it, I can not answer your question correctly and with confidence. I advise you to set an appointment with a neurologist in this regard. The drugs that you consume are antispasmodic and anti-anxiety. They cannot alleviate either muscle twitches or muscle cramps (the painful muscle contraction that awakens you). Anti-GAD antibody is a screening test for a specific disease that can cause frequent painful muscle contractions throughout the body. While waiting to visit a neurologist, I recommend you use calcium supplement, magnesium supplement, and vitamin E (at least 400 units/day). These supplements may help to decrease the painful cramps, but not the twitches. Consult your doctor and take the medications appropriately.
