Hi doctor,

I had hard stools four days back and after that, I am having enormous pain while passing stool every day. I consulted a doctor and he prescribed Ano-metrogyl cream. I used it three to four times a day but there is no relief. I drink Softovac powder at night. My situation is not getting better.

You have developed acute anal fissure due to hard stools and so you are having pain due to it. There are two ways to resolve it. You can use local cream to have relief from pain and to relax your sphincter. You can take medications to soften your stools. I am suggesting some medications and kindly follow them to have some relief. Take plenty of water and avoid having hard stools.

Treatment plan

You can use GTN (Glyceryl nitrate) cream. Mix it with Ano metrogyl cream in equal quantity and apply it four times daily locally. You can take syrup Lactulose 30 ml at night. Titrate your dose according to your stools and you should pass two to three soft stools after taking it. Continue taking Softovac powder along with it.

Preventive measures

Avoid having hard stools.

Regarding follow up

Have a follow-up after three days to see for improvement. If there is no relief, we can change the treatment and plan for future prevention.
