Hello doctor,

I am a 30-year-old male and I have been suffering from genital itchiness and foreskin tightness for years. I consulted a doctor and was tested for HIV which was negative. The doctor suggested it could be STD-related and prescribed medications including an ointment total derm plus. At present, I am using sodium fusidate ointment. Kindly suggest an ointment that I can use as the prescribed ointment is unavailable.

The STD-related condition can be balanitis, phimosis, or paraphimosis.

This condition could be caused due to the following reasons:

  1. Improper hygiene.
  2. Unwashed soap on the penis.
  3. Scratching and rubbing the affected region.
  4. Infection.

I suggest you follow a few instructions mentioned below.

Any of the following ointments can be used in place of

  1. Clobetasol ointment.
  2. Gentamicinointment.
  3. Chlorhexidine Gluconate ointment.

Oral antibiotic medication and anti-fungal with steroid combination.

Drink plenty of water.

Kindly follow up if the symptoms do not resolve.

Thank you for the reply,

I feel pain and discomfort in my penis after sexual intercourse. I also noticed pimples appearing on my foreskin. I am very concerned about that. Can you suggest any other ointment or antibiotics, as my case is STD-related?

In balanitis blotchy rashes or spots may present as typical features. Tightening of the foreskin can commonly cause pain during intercourse. If pain is severe you may need circumcision (cutting of foreskin). Circumcision is a minor operative procedure in which the foreskin is removed by the surgeon.

I have never felt the tightness of the foreskin before, and I strongly think it is due to the STD-related disease. I do not think I need to get a circumcision. I am concerned about the small pimples appearing on my foreskin. Kindly request you to prescribe me some medicines or ointments.

I suggest you to get tested for HIV, as symptoms of HIV (human immunodeficiency virus) develop over a period of eight to ten years after getting an infection.
