How do I permanently get rid of blackheads on my nose and chin?

Blackheads (Comedones) are skin pores that get clogged. There are no definite treatment for a permanent cure from it although you can treat the present ones and also follow steps to prevent it. If you have an oily skin, you are much more prone to developing blackheads and you can follow some simple steps: Avoid oily foods, fatty foods, dairy products, avoid oiling of the face, avoid constant touching of the face with unclean hands. As for the blackheads, you can meet a dermatologist who would extract the comedones for you. To prevent further comedones you can use face wash that consists of salicylic acid twice/thrice daily, apply a suncreen with a gel-base, try retinoid cream (with a low percentage after consulting with a dermatologist after seeing the pores under dermoscope).
