Hello doctor,

My last menstruation happened 45 days ago. I did a home pregnancy test one week ago, and the result came back positive. But I had spotting 40 days ago. The beta hCG, which was 115 on my first test reduced on a repeat test. I started bleeding today, which I guess to be a miscarriage.

I need to know whether this will naturally clear my uterus or what has to be done next.

Please guide me.

I understand your concern.

Your bleeding is because of spontaneous abortion. This is characterized by heavy bleeding with or without clots for one day, followed by reduced bleeding and then stopping bleeding. This can progress into a complete abortion, leading to the passing of the complete tissue and residues of conception, which clears the uterus.

Otherwise, it may progress into incomplete abortion where bleeding may start, but it may not stop and may not lead to the complete passing of products of conception. Hence, if your bleeding is very heavy and not stopped even after 24 to 48 hours, if there is fresh red bleeding or any symptoms of blood loss like tiredness or dizziness, you should visit a doctor immediately.

It is highly recommended to do an ultrasound scan in both cases of abortion.

Thank you so much for all the clarifications.

I still have bleeding, but it is not heavy. And I have on and off pain in my abdomen and lower back. Should I go for a repeat beta hCG, or will a scan be sufficient?

I would recommend a scan, as hCG (human cryogenic gonadotropin) will be raised for up to 15 days even after complete abortion. Hence it is not very reliable.
