Patient's Query

Hello doctor,

I am 37 years old. My periods are close to two weeks now, and I am still bleeding, sometimes in clots. Please help.

Answered by Dr. Reetika

Hello,

Welcome to icliniq.com.

I can understand your concern and will surely help you.

May I know a few details like,

  1. Have you done any rigorous exercise anytime in the recent past?
  2. Are you breastfeeding?
  3. Is your thyroid normal?

I suggest you do a urine pregnancy test at the earliest, and if it is negative, we can start with medication.

I suggest,

  • Trapic MF (Tranexamic Acid and Mefenamic Acid) twice daily for three days.
  • Tablet Deviry (Medroxyprogesterone Acetate) -10 mg twice daily for ten days.

Do not worry. You will be fine. Share complete medical history before taking any medication. Detailed information will help me to assess your condition better. Feel free to ask any questions. I am always there for you.

God bless.

Patient's Query

Hello doctor,

Thank you for the reply.

My thyroid is normal, and I am not breastfeeding. However, I had intense sexual activity in the recent past, which possibly is a reason. I had taken an hCG test 15 days back, and the value was 0.200. Kindly suggest if any other urine pregnancy test is required.

Hello,

Welcome back to icliniq.com.

I request you to please share the report and also tell me how many days after the intercourse you got the hCG test done, as their values are related to how early or late you have done the pregnancy test.

You can do the urine pregnancy test. Feel free to ask any questions.

God bless.

Patient's Query

Hello doctor,

Thank you for the reply.

Is the bleeding related to pregnancy? Instead of a urine test, can I test using pregnancy test kit? I got the test done two days after intercourse. I do not want to be pregnant. I am attaching my report details.

Hello,

Welcome back to icliniq.com.

First of all, do not worry. I am there to help you.

I have gone through the report shared by you (attachments removed to protect the patient's identity). You are not pregnant now, but it was just two days post intercourse, and ideally, for the accurate results, one needs to wait for at least two weeks post intercourse. May I know whether the intense sexual activity was protected or unprotected?

You can use the pregnancy test kit, preferably with the first urine of the day. If the urine has passed already, try to do it before your intake of routine fluids like tea, coffee, water, etc. Adhere to all guidelines written on the package of the pregnancy test kit, especially the timings for checking with the equipment and click pictures. This is because timing is critical, and with time-lapse due to various reactions, the color of the strip could change. So take the picture as per the actual timeline of the kit you are using.

Do not worry. Most likely, you are not pregnant as you have so much bleeding, but we should know about our body before starting medication and not end up using incorrect medicine for a wrong diagnosis.

If you can get blood hCG test results, it is even better. Feel free to ask any questions or to share any additional information.

God bless.

Patient's Query

Hello doctor,

Thank you for the reply.

I will get the kit test done. My period started with dark brown spotting for four to five days and then heavy bleeding for almost ten days. Now there are big clots till today. Is this due to hormone imbalance? I have never had longer than four-day periods before this. In the last ultrasound scan, there were two small fibroids. There is a slight pain in the feet too. I will get back to you after the tests.
