Hello doctor,

I have extreme mood swings that alternate between high and low every few weeks. When I am happy it feels like magic, I feel wired up as if I have just taken 50 cups of coffee. It is great because I feel positive and a lot more energetic than usual. Unfortunately, while being hyper or euphoric feels brilliant it sometimes comes with serious consequences. I get impulsive and impatient, and I get into massive arguments and physical fights with other people (not always because I am angry but because I think it is fun or exciting to have someone twice my size to wrestle with me), I know it is destructive or reckless but it is entertaining and it is really dangerous. I also tend to escalate quickly from one extreme to the other, one minute I am like everything is awesome and the next I am trying to kill someone because the voices command me to. At the opposite end of the scale, I get depressed and suicidal. I tend to want to injure myself and I often think about killing myself. What is happening to me? I have a family history of bipolar and other mental illnesses. Also, I have been diagnosed with borderline disorder. I am taking 10 mg of Aripipraizole. I got an MRI and genetic testing done. Kindly suggest.

I have read your query, and you have given a very straight and elaborated history, which suggests that you are having rapid mood swings which is a sign of bipolar affective disorder (BPAD). But this illness is curable. You are having ultrashort period mood swings in which the prognosis is good and easy to treat. Medicines like Valproate (Sodium valproate and Valproic acid) and Lithium (Alkali metal) will do the best in this condition. Aripiprazole (Dichlorophenyl and Dihydrocarbostyril) is fine but after using a mood stabilizer, it should be stopped slowly as a mood stabilizer will not be sufficient to deal with your condition. Psychoeducation and counseling are also important in BPAD. Since you are 18 years old, your symptoms will subside with growing age. And yes your personality is having borderline traits but milder in grades. I hope this helps you.
