Hello doctor,

Recently I have had spots on my penis that have become noticeable. I have been diagnosed with ureaplasma and kidney stones. I had a computed tomography scan today because of the bumps on my genitals for the past month. I have a lousy tingling sensation in my urethra even after taking tablet Azithromycin for an entire week. I have tested negative for gonorrhea and chlamydia. I also have a sebaceous cyst on my right kidney. But my query is, do those bumps look like herpes?

I read your query and can understand your concern.

From the attached images (attachments removed to protect the patient's identity) it is not herpes infection. Those are the Fordyce spots, which form on the penile skin and require no treatment because they usually go away on their own.

Can you please tell the difference, and are you sure they look like fordyce spots and not a herpes infection? What about my urethritis? It is constantly tingling at the tip of the penis. I was diagnosed with ureaplasma but have taken an entire course of antibiotics. Also, what could it mean if the moment I drink alcohol, it burns when I pee?

Yes, I am sure it looks fordyce spots. If burning in urination is affected by hydration status, it is not due to infection. Concentrated urine can cause a burning sensation.

Well, ureaplasma will cause a burning sensation rather than a tingling sensation in the penis and will not reduce with hydration even after treating a mild infection. Inflammation will be fine on its own. However, sometimes the sensation annoys a lot, so we prescribe anti-inflammatory medications to reduce symptoms until they recover.

Fordyce spots are self-limiting. They do not require any treatment and go away on their own.For the tingling sensation in the urethra, you need to have another course of antibiotics. I suggest you take tablet Ciprofloxacin 500 mg twice daily for two weeks. Consult your doctor, discuss with them and take medicine with their consent.

There is no such relationship between alcohol and burning. As I said, burning is on and off according to hydration. If the color of urine is yellow and burning, or it is pale with no burning, it is probably due to water intake. But burning must be treated as prostatitis with a prolonged course of one month of antibiotics and alpha-blockers if it is continuous.

Same symptoms don't mean you have the same problem. Consult a doctor now!
