Patient's Query

Hello doctor,

My uncle is suffering from HIV and his CD4 count is 57. At what stage is his disease at present? He is not feeling hungry and his body temperature is high. He is taking tablets Instgra 50, Telura, Albavir, and Bactrim DS. Kindly suggest a few tips to improve his count.

Hi,

Welcome to icliniq.com.

With passion, commitment, and experience, I hope I will be able to provide you with a top-class medical opinion.

My inference is as follows:

  1. As per the stage determination based primarily on CD4 (clusters of differentiation 4) count, your uncle is at stage three of the disease.

  2. Based on the prior medications, your uncle has received first-line ART (antiretroviral therapy with the tablet Telura. Please let me know how he had taken the same.

  3. The attachment shows that (attachments removed to protect the patient’s identity) the treatment given to him is two or three lines of ART. Also, kindly let me know when these drugs were prescribed and if is he taking these regularly.

  4. Has he received any other antiretrovirals besides these?

  5. From the limited history that you have provided, he seems to have developed treatment failure with the first line of drugs and then a switch to ART has been done. Also, let me know the HIV viral load if that test has been done.

  6. Regarding the loss of appetite and high body temperature, please note down the body temperature and get back to me. He might have acquired opportunistic infections.

In individuals who regularly take their prescribed antiretrovirals (not missing even a single pill in a month), the CD4 count is expected to improve within six months.

I hope you had a pleasant experience at icliniq.

Thank you.
