Hi doctor,
Please help me in understanding a few things. I had one sexual incidence with an unknown HIV (human immunodeficiency virus) status lady. Although it was protected, I do not know the status of the condom after sex. Then, I had 2 CMIA tests (chemiluminescent microparticle immuno assay). First test by 24.5 weeks and the second test by 25.5 weeks. Both were having the index of 0.14 and 0.08 with nonreactivity. I am attaching the reports for the reference.
Are my reports conclusively negative? Do I need any further test? Is CMIA equally good as 4th generation ELISA? In my case whether it was tested for both Antibodies and P24 Antigen or not? What is the false negative chance using CMIA? Is CMIA better than Rapid kit test?

CMIA tests done at 24.5 and 25.5 weeks are nonreactive. It implies that you are not infected. CMIA TEST is only 4th generation tests, and Elisa tests are 3rd generation. The only test which is better than CMIA duo is RNA PCR TEST, and however, it is not required in your case. CMIA detects both p24 and antibodies. It is to find out infection at earliest possible days like say three weeks after exposure. And this test is conclusive after 12 weeks so as in your case. It is 99.9% accurate and false negative chances are low, and after 12 weeks those are negligible. CMIA is definitely better than rapid tests.

P24 antigen assay test is only useful during an early stage of infection where the human body does not produce enough antibodies, and this assay can decrease window period, so one uses this method to detect infectivity during the early test. Antibody detection is the standard test to detect HIV as the p24 antigen levels decrease after some duration. So, as in your case, you have done analysis after 24 weeks by CMIA duo as the titers are par below infectivity range.

I do want to understand that is it possible that antibodies are not formed even at 25 weeks. If yes, that antibodies are not formed even then P24 antigen will be present. One more thing, is there any specific condition or time when neither of one is detectable through this CMIA method?

What you asked is the rarest, rare condition. In a normal person, antibodies will be definitely produced by B lymphocytes against a particular foreign antigen, except for certain conditions that lack hypogammaglobulinemia. At the early stage of infection, there is called a window period in which you can measure both antigen and antibody.

Yes, you can. CMIA test is very much conclusive at three months. As you have done tests at 24th week, it is conclusive.
