What is the treatment plan for an elderly with a 15 % working heart, reduced sugar, blood pressure, and increased creatinine levels?

Patient's Query

Hello doctor,

My father's 2D echocardiography shows only 15 % of his heart is working. Currently, he is in MICU for the last day, and for the last three days his sugar is getting very low frequently around 50 to 80 mg/dL, and his blood pressure is also low at 60 to 100 mm of mercury. Creatinine is high around 5.2 mg/dL (this problem arose last month). His heart bypass surgery happened ten years back. Please help. Thank you.

Hello,

Welcome to icliniq.com.

I read your query and understand your concern. I understand your concern regarding your father's condition. I have gone through the summary and reports (attachments removed to protect the patient's identity) you have provided. The situation here is multiorgan dysfunction including heart and kidneys along with systemic infection. Due to poor cardiac functioning, the kidneys are further compromised. Hypoglycemia may be attributed to multiple factors here like kidney dysfunction, sepsis, or medications. He needs ICU (intensive care unit) care, because he has an increased tendency of retaining fluid again and again due to the poor functioning of the heart, it is not pushing the blood forward and kidneys are not able to remove extra fluid from the body. The medications that he is on, are accurate enough to tackle the current situation, we can only wait for improvements here. Is he having fever spikes as well? Thank you for your trust in me. At icliniq, you will always get the best opinion from more than 3500 medical experts.

Patient's Query

Hello doctor,

No, the fever did not come for the last ten days from when he is admitted to the hospital. Kindly suggest any experienced cardiologist with whom I can show the reports and 2D echocardiography and get some advice on how to proceed. Thank you.
