Hi doctor,

I have symptoms of dizziness and headaches, and I feel a kind of confusion and pressure in my head. I also think that my problem-solving and cognitive function is diminished. I have been experiencing the symptoms for the past year. I have undergone diagnostic tests like MRI, cognitive function test, sleep apnea test, a full panel of biochemical and blood tests, vitamin B12, cardiac biomarker test, cardiac stress test, Holter monitor, 5-day event monitor, echocardiogram and no significant abnormalities were seen in all the tests.

I experience a feeling of electric pulsation in my head, which significantly affects my concentration, vision, and driving. These symptoms are more frequent now.

I doubt if it could be due to intracranial pressure, tumor, seizures, epilepsy, neurodegenerative diseases, aneurysm or panic attack. I am anxious about my condition. Please advise if any further investigations need to be done for the same.

I understand your concern.

I have carefully considered your question and would like to explain that your symptoms could be related to seizures or a panic disorder, or it can be an inner ear disorder.

I want to exclude cardiac issues, intracranial hypertension or hypotension, any brain tumor, aneurysm or infection, or neurodegenerative disorder, as all your tests turned out to be normal.

I would recommend performing an EEG (electroencephalogram) to investigate seizures, consulting with an ENT (ear-nose-throat) specialist, and perform labyrinthine tests to examine for an inner ear disorder.

If these tests result normal, you should consider anxiety as the main cause of your complaints.

Hi doctor,

Thank you for the reply.

I also want to inform you that I cannot concentrate while driving. I feel like the lights go on and off, and I lose track of the surroundings. Though the episode lasts a few seconds, I experience these episodes continuously, which may even last for 7 hours.

I have also been suffering from headaches in different parts of my brain.

Please advise and suggest.

I understand your concern.

The duration of such episodes is not typical of seizures because seizures last for some minutes. The loss of vision could be related to low blood flow to the brain. But, the brain MRI is normal, and no evidence of brain ischemia is revealed.

The fact is that these symptoms occur each time you drive and could be related to a psychological problem. So, it seems that anxiety plays an essential role in this condition.

Before coming to this conclusion, I recommend performing an EEG (electroencephalogram) for seizures, a doppler ultrasound of the carotid arteries to exclude possible narrowing, and a head-up tilt test for orthostatic hypotension. Orthostatic hypotension is a medical condition caused by a drop in blood pressure values and, thus, the blood supply to the brain during sitting or standing up differs compared to lying down.
