Hello doctor,

I wanted to obtain your second opinion regarding my abdominal pregnancy. I am set to receive laparoscopy to investigate but wanted to reaffirm my prior reports due to the Obgyn's disbelief that what is being investigated are actual twin fetuses (which still move every day). I was also confirmed with positive urine and low levels of hCG in my blood. The doctor believes it is an early pregnancy miscarriage. Still, I would like to reaffirm that I am currently experiencing a prolonged ectopic pregnancy and need immediate attention to my fetuses' health and safety. Please feel free to look at all the images and files provided.

Thank you very much for sharing all the images (attachment removed to protect patient identity). I carefully reviewed them all. Let me start telling you about ectopic pregnancy. Ectopic pregnancy means when a fertilized egg grows outside the uterus.

There are a few types of ectopic pregnancy intra-abdominal, ovarian, tubal, and cervical. The images show no intra-abdominal or ovarian, or tubal pregnancy sac to explain the movement you feel. There is a large cystic lesion in your cervix. However, it is most likely a simple nabothian cyst because it lacks a blood supply. I would still follow that cervical cyst.

The other important thing to note, sometimes when there is a miscarriage or very early ectopic pregnancy, radiologic imaging may not show it.

I do not suggest laparoscopic evaluation at the moment. Follow up blood hCG (human chorionic gonadotropin) levels. If it continuously decreases, there is nothing to worry about. If it continuously increases, repeat imaging.
