Hi doctor,

I am 23 years old. I was masturbating for about 13 years. I was seriously addicted to masturbation for almost six years. It has been just a week I finally got a strong determination to quit masturbation. But now I am very tensed about my sexual life. I do not last even a minute since, as much as I can remember the past four years. It just takes 20-30 seconds to reach orgasm. Probably 30-60 seconds when I try to control through masturbating slowly. I have never experienced sex yet. How can I recover from my sexual condition faster? I am also taking Paroxetine 12.5 mg daily.

Is it the generic names? If not, please tell me the generic names of these drugs, so I can find them easily in my local pharmacy. How many doses should I take every day (for example, day and night, daily or not) and, for how many days should I take? Additionally, I am suffering perhaps from prostate problems too, due to this. It takes more than 2 minutes to clear pee. Thanks.

I see that you are taking Paroxetine. Coming to this point, I would first recommend increasing the dose of Paroxetine to 50 mg daily. It is equal to Imipramine in delaying ejaculation. I would also recommend performing pelvic floor exercises. Weak pelvic floor muscles might impair your ability to delay ejaculation. Pelvic floor exercises (Kegel exercises) can help strengthen these muscles.

To perform these exercises:

Find the right muscles. To identify your pelvic floor muscles, stop urination in midstream or tighten the muscles that keep you from passing gas. These maneuvers use your pelvic floor muscles. Once you have identified your pelvic floor muscles, you can do the exercises in any position, although you might find it easier to do them lying down at first. Perfect your technique. Tighten your pelvic floor muscles, hold the contraction for three seconds, and relax for three seconds. Try it a few times in a row. When your muscles get stronger, try doing Kegel exercises while sitting, standing, or walking. Maintain your focus. For best results, focus on tightening only your pelvic floor muscles. Be careful not to flex the muscles in your abdomen, thighs, or buttocks. Avoid holding your breath. Instead, breathe freely during the exercises. Repeat 3 times a day. I understood about the taking of Paroxetine 50 mg. It was advised by my psychiatric doctor 12.5 to 25 mg. However, I took 20 mg at first and half of the pill the next day. I felt like vomiting and sleepy. Then she advised me to take plain Paroxetine. So I need to ask you, would 50 mg make me feel weak or sleepy or any other side effects? Plus, how many days I need to take this pill? Should I not take Imipramine anymore? If I face problems regarding understanding the pelvic exercises, I will contact you for help.

I would recommend increasing the dose of Paroxetine gradually by 12.5 mg every week and achieving the dose of 50 mg after a month. This will help avoid the adverse effects that you have experienced. Paroxetine is better than Imipramine (less adverse effects). For this reason I would recommend taking just Paroxetine with gradual increase of the dose. Regarding the exercises, you can find a lot of information about them on the internet.

So you want me to take Paroxetine for four weeks. Shall I take 12.5 mg from the next week or 50 mg one, right? In total, how many months should I take this tablet then? And I have seen on the internet that this tablet is helpful for social anxiety or disorder, oral disorder etc. So can this pill recover my situation in case of lasting longer to reach orgasms?

I recommend taking 12.5 mg daily for one week, then 25 mg daily for one week, then 37.5 mg daily for one week and then 50 mg daily for the next three to six months. Yes this is a very good drug for anxiety, social phobia, etc. Hope you will find this answer helpful.

Hope you are doing well.

Doctor, since you and my psychiatric advised me to take Paroxetine CR (controlled release preparation), I searched for it in my local drugstores. But did not find it. They have Paroxetine hydrochloride 20 mg and 10 mg, which is not controlled release preparation I think, the maximum dosage they have is only 20 mg. Please check the attached file (attachment removed to protect patient identity), I have given the available Paroxetine details. I took the available Paroxetine 20 mg, and I had similar complaints, which I had informed about to you earlier. So I informed about it to my psychiatrist. She advised me to take Paroxetine CR preparation instead of plain Paroxetine. Now please advise me what to do?

Do we have any other similar medicine for late ejaculation and prostate problem?

In my opinion, all the above formulation are good for you. I recommend taking first 10 mg pill and take half pill (5 mg) for a week once daily. The next week take 10 mg daily, the next week 15 mg daily, the next week 20 mg, then continue with 25 mg and increase this way up to 35 mg daily. Then stay at this dose for a month and see if you feel better.
