Hello doctor,

I had an abdominal ultrasound scan done today and report shows that my liver is mildly enlarged and shows diffuse coarse structure. No focal lesions noted and span is about 15.5 cm in the mid-clavicular plane. I am a known diabetic for the past 6.5 years, and even has some blood pressure around 130/80. I am using Betaloc-50 for that. I am having acidity problem regularly. 13 years back, I had a mild fatty change.

Such findings in ultrasound represent fatty liver disease. The causes can be alcohol, non-alcoholic fatty liver disease, chronic hepatitis B, and C. In such case, I would advise you to get liver function test, anti-HCV (hepatitis C virus) antibody, HBsAg, In case these reports are normal, and you do not consume alcohol, it means you have a non-alcoholic fatty liver disease. Get the tests done and revert back with reports.

Is my condition serious? Any damage to the liver has happened? I just want to know in detail. HBsAg is negative and even HCV is negative. I will undergo liver function tests also.

The good news is that your Hepatitis B and C are negative (attachment removed to protect patient identity), so your fatty liver is because of alcohol or non-alcoholic fatty liver disease. As per your HbA1c report, you have diabetes according to the standard laboratory criteria. But the laboratory values mention your HbA1c within normal limits. So, I would advise you a fasting and two hours postprandial blood sugar levels. For fatty liver, I would advise you a strict diet control with regular exercises.

My fasting blood sugar is 100 and postprandial is 140 and the lipid profile I have attached in the previous query, the values are normal. Please have a look at those and give your valuable suggestions. I am not an alcoholic. So how should I control my fatty liver?

I am glad to know your well-controlled blood sugar levels. If you are taking any medicine for that then let me know. For fatty liver, you just need to do regular exercises and walk. Keep your blood sugars well controlled. No particular medicine is required.
