Hello doctor,
I am a 31-year-old unmarried male with a history of potential low-risk exposures to sexually transmitted infections. Two years ago, I had one instance of unprotected sexual contact. Five months ago, I had a non-penetrative sexual exposure involving manual contact with a fully clothed individual who had no visible cuts or breaks in the skin.
Following the latter exposure, I underwent testing for HIV 1 and 2 (fourth generation), hepatitis B and C, and syphilis at 3.5 months, all of which were negative. Due to ongoing anxiety, I have also had fourth-generation HIV tests approximately monthly over the last two years, with the most recent test last month, all non-reactive. I have no symptoms.
Could you please advise if additional testing, such as HIV PCR (Polymerase Chain Reaction) or NAT (Nucleic Acid Amplification Test), is recommended at this point?

I can totally understand your anxiety, and it is very much normal. The fear of HIV is quite disturbing. HIV infection has a window period of 12 weeks, during which the test results can be false negatives. However, beyond 12 weeks of the last exposure, the result can be taken as conclusive. Also, a handjob is not considered to be a high-risk exposure and has almost zero chances of HIV transmission.
Considering your history and the duration of tests done, the results can all be taken as negative and conclusive. Therefore, you do not need any further testing.
Meditation, involving yourself in physical activity, would help to relax your mind and keep it away from thoughts of HIV. If the doubt persists, you could consider talking to a counselor.
