Hello doctor,

Last month, my boyfriend was fingering me on the 11th day of my cycle. While doing it, he got a call, and he went to take it. I do not know if he touched his penis, the damp part near his pants that gets saturated with precum or sperm secretions. Most importantly, I do not know if he ejaculated or not. So after two to three minutes, he came after attending the call and continued fingering me. We both were fully clothed, and no genital contact, rubbing, or penetration happened. He slid his finger inside my pants and panties and started rubbing my clitoris. He did not enter the vaginal hole. I have PCOS and irregular cycles. I have not got my periods for the last nine months but only spotting for the last six months. I started taking Dronis 30 the last month from the 12th day of my cycle as I had brown spotting from day one to the fifth of my cycle, prescribed by my gynecologist. What are the chances of pregnancy? Kindly help.

I suggest there are no chances of pregnancy. You have PCOS (polycystic ovarian syndrome), and periods occur at irregular frequencies, so there is no chance of ovulation so early. Your history says that you had the tablet Dronis (estrogen and progesterone), which stops any chance of ovulation. So if no egg is released, there is no chance of pregnancy. You have not had penetrative sex, so the chance of active sperms in the vagina is nil. Even if he had a few dry sperms on his finger, they would not be enough to swim up. So there is no chance of pregnancy.

Thanks for the reply.

I started taking the tablet on the seventh day of my cycle, and this fingering thing happened on the 11th day of my cycle. So for Dronis 30 to act as OCP, it should be taken from the first day of the cycle itself. But I started taking it from the seventh day. So, will that work? Also, on the seventh day of my cycle, I did a scan that showed the size of the uterus and attached the reports. The largest follicles in ovaries are 5 mm and 6 mm; I read that these large follicles only become an egg and will be released, which causes ovulation. So even though I started taking the tablet Dronis in the middle of the cycle, will that help prevent ovulation? I have not missed any doses, and I take pills at the same time every day. I have had vaginal discharge that is white, thick, and creamy. I can sense it coming from my vagina. It was not a lot and a little, but I would always feel it on my panties, wet and something coming out of my vagina. I had breast soreness, abdominal pain, pelvic pain, and back pain on the fifteenth day of my cycle. And I thought it was implantation bleeding. I am so tense since then. I was so bloated even if I only ate an egg for breakfast, and I could not even eat lunch, a few snacks, and I felt full. Why is this happening? Are these pregnancy symptoms or other reasons? Kindly help.

Tablet Dronis (estrogen and progesterone) is to be taken before ovulation, which can never occur so early. Scan reports are normal (attachments are hidden to protect the patient's identity). If you start Dronis, all those changes in discharge and breast will occur. That is normal. You have no chance of pregnancy.

I stopped my last two pills, which were yesterday and today. Because yesterday I had a headache that I could not bear; my gynecologist said to stop the pill if I have a severe headache. Only the last two pills are left. I never missed any dose even though I had side effects like headaches which are moderate. I continued taking the pills religiously till the last month. Will that cause ovulation? Will fingering make me pregnant by missing these two doses?

Ovulation is not just a daily process. All the series of hormones for the full cycle are needed for normal ovulation. You have already disturbed that balance by taking the tablet Dronis 30. Missing a day or two of the last pills will not affect ovulation.

I am sorry for being paranoid. I could not stop stressing about it. Before consulting, I searched so much, which made my mind go crazy. I am new to these sexual things, so I do not know much about anything. Also, he said he could not remember if he touched his private parts. I have seen him keeping his phone on a TV stand and coming to me. So, can this have any chance of pregnancy? Today I have seen a very light brown discharge coming from my vagina only once, that is the only time I noticed it, and after that, I had a white discharge. I will attach a picture of the discharge. I still see no chance of pregnancy from the history provided. Nothing matters if you do not have an egg in your tube. So the only way you can have confirmation is to have a serum beta hCG (human chorionic gonadotropin) level blood test after 14 days of this sexual episode. Even the earliest pregnancy can be detected.

Thanks for the reply.

After two days of stopping the last two pills, I had a dark brown spotting. And it gradually turned into light bleeding with clots and cramps. On the next day, it filled up my pad with dark red and dark brown with clots and cramps. And on the third day, it is light bleeding with dark brown and dark red and is pretty thick, more than a spot, and less than a heavy flow. And I think it will stop today itself. Does it rule out any pregnancy? Or is this any other cause?

When you see the inner lining of the uterus bleed out into the pad, how can there be pregnancy? As I mentioned, why do you not have a serum beta hCG blood test from any laboratory or hospital if you still doubt pregnancy? Any value less than ten units indicate a negative pregnancy.

On the 11th day of the cycle, my boyfriend fingered me where no penetration was involved; we had our clothes on during that time. And I had my period a few days after that with brown and red bleeding. I have been having abdominal pain and back pain on the right side, which is unbearable. My doctor suspected kidney stones took my urine sample to rule out any infections and referred me to a scan. I have attached the scan reports for you to check. Beta hCG is negative with less than 0.100 mIU/mL. She said since beta hCG is negative, it is not a sac, but it can be a cyst, and she said there is no need for another scan as it is negative. Even the radiologist was in doubt when he asked me about my situation. I said the whole scenario, and he said it was best to rule out any pregnancy. Both the UPT and beta hCG are negative. Then why is the report had a gestational sac? Am I pregnant or not? Should I go for another beta hCG test to be sure? And also, the beta hCG is taken on the 23rd day from the sexual date. I am assuming it should be accurate enough to be negative. Or am I wrong? Please kindly guide me.

There is nothing problematic in this. As expected, you have a polycystic ovary, not pregnancy. You have renal stones in the right kidney. Nabothian follicles mean infection in the cervix. Usually asymptomatic. I suggest you have antibiotics as per your gynecologist's advice. No need to follow up.

According to my scan report, I would like to know the size of the cyst. Is that big? My report shows a scan photo of this nabothian cyst that looks exactly like a gestational sac. And is that a cancerous cell? Will I have any problems in the future? I do have some symptoms I do not know if that is related to cysts or not. Symptoms that I have are pain inside the vagina, pelvic pain, pain in the whole left breast, and pain near the belly button. This pain near my belly button is so sharp that it will be there for 24 hours, but it is not that unbearable but uncomfortable and affecting my daily life. It is like something is pulling inside. When I touch where the pain comes from, it pains around the area. Is that related to this cyst or other reasons? Sometimes vaginal discharge is low, and sometimes it is dry. What can cause that cyst? Why was it not visible in the previous scan I took a month ago?

Let me start by saying this is not cancer. Infection from semen or vagina or any tampons used can enter the lining of the cervix or mouth of the uterus and get stuck within the lining. They will stay there forever. The lining covering becomes thicker and dormant. They cause no symptoms. Many times we get pimples on our faces. Over months, they do not rupture but can be felt as thickening if we touch them. They are harmless. We do not see the size of those pimples, same with these cysts. Nobody measures them as it does not affect prognosis. Maybe your previous radiologist did not think it was important. The rest of the symptoms are unrelated.

I hope you remember my query. As I said, I have done my beta hCG test, and it was negative. And on the same day that my bleeding started, which I can say is more than a spotting like I have been having for the last three months. It lasted for five days. And I have no signs of ovulation happening. So will that affect me or my fertility in any way due to this and the off-rise values of hCG? Or do you think it might be ectopic pregnancy because I have been having some shoulder pain?
