Patient's Query

Hello doctor,

I had vaginal sex with a sex worker with a condom. Later I found that the condom covered only 60 % of the penis. After three weeks, I have developed the following symptoms: sore throat, penis irritation and tingling, frequent urination, white tongue, arm weakness, body weakness, weight loss of about 7 lbs within two weeks, tingling sensation in legs, hot back and lower foot, body aches, joints pain, hip and thigh pain, lower stomach pain, red circular skin rashes, hemangioma like structure in the skin which is a permanently present, pimple-like structure in the skin which comes and goes away, neck discomfort, pain in the left shoulder, acute testes pain, and loose stool for a week. After a month of having those symptoms, some symptoms have gone away. Still, I have body weakness, a white tongue, and permanent spots on my skin. The same symptoms also persist for my wife after two weeks of my symptoms. Am I infected with HIV? Is my symptoms related to HIV? Is the above tongue image indicate candidiasis or oral hairy leukoplakia?

Answered by Dr. Rajesh Deshwal

Hi,

Welcome to icliniq.com.

I thoroughly read your query and understand your concern.

No, none of the symptoms are related to HIV (human immunodeficiency virus). They are all symptoms of anxiety and panic. I saw the photograph (the attachment is removed to protect the patient's identity), and no, it is not candidiasis or oral leukoplakia. I suggest you take a 4th ELISA (enzyme-linked immunosorbent assay) for HIV to allay your fears.

Hope this helps.

Best wishes.

Patient's Query

Hi doctor,

Thank you for the reply.

Actually, after having sex, I never thought of such possible infections. However, after such unique symptoms arose, I suspected that it could be HIV. My wife is also having the same symptoms as mine, and recently, she had a fever for a week despite taking medicine. Somehow, I had contacted that sex worker for a test, and it was found that there were no visible nerves to collect a sample as she had been injecting drugs for many years. Later somehow, the sample was collected and tested for rapid HIV test within 45 days of exposure, and it was a negative result through Oraquick's advanced rapid test kit in a private laboratory. Is the p24 antigen-antibody test by CMIA the same as the p24 antigen-antibody by ELISA? Every morning, I had to clean my tongue and had to scrape out white particles. After removing white particles, hairy white patches cannot be scrapped out. And its smell is also unusual. Please help.

Hi,

Welcome back to icliniq.com.

I understand your concern.

If you have been tested after 45 days, in all probability, you are negative. You can ask for 4th generation ELISA, which includes p24 antigen, too.

Hope this helps.

Regards.

Patient's Query

Hi doctor,

Thanks for the reply.

I did p24 antigen-antibody by CMIA in 44 days of exposure and 110 days of exposure, and it came out nonreactive for both antigen and antibody. Is ELISA more reliable than CMIA? As per her narration, one HIV-positive worker has also worked in their group, and she has shared the same needle with her for injecting drugs. Does the fourth-generation p24 antigen-antibody by CMIA have a longer window period than the fourth-generation p24 antigen-antibody ELISA? The above image is taken this morning. The white tongue persists for four months. That spot in the arm developed four months ago, and it still persists. Are all the symptoms related to HIV?

Answered by Dr. Rajesh Deshwal

Hi,

Welcome back to icliniq.com.

I understand your query and concern.

If your p24 antigen assay is negative at 110 days, you are negative for HIV. These symptoms are not related to HIV. Relax and carry on with your usual life.

Hope this answers your query.

Kind regards.

Patient's Query

Hi doctor,

Thank you for the reply.

What could be the possible reasons for my symptoms? I have had those symptoms for the last five months. Now, I have been having lower back pain, too, for the last two weeks. I have been having pain inside the left shoulder area, and it really irritates me. I did HIV 1 DNA detection qualitative test within four months of exposure, HIV 2 RNA (ribonucleic acid) detection PCR (polymerase chain reaction) qualitative in four months and seven days, HIV 1 RNA detection qualitative PCR in four and half months, HIV 1 viral load by Taqman RT PCR in four and half months. All reports came out, and the target was not detected. Also, I did a rapid antibody test this month, which was also negative. But I am still having the symptoms. What to do?

Hi,

Welcome back to icliniq.com.

I understand your query and concern.

I have already given you the reason for your symptoms in my first answer.

Patient's Query

Hi doctor,

Thanks again.

I did not have anxiety before the symptoms appeared. After those symptoms, I visited the doctor, and after a physical examination of my penis rash, white tongue, and skin rashes, I was given a Doxycycline initially. After taking Doxycycline, my symptoms persisted. Then he prescribed me anti-fungal and anti-parasite medications. Even after taking those medicines, new symptoms are being added with each passing day. Is my tongue normal as per the image? After how many days of exposure to HIV, does two qualitative PCR test gives a confirmed result? Actually, there is no testing center in my city for 4th generation and PCR tests. So a sample is being sent from my city to another, which takes about 15 hours to reach a place. Then from there, the sample is sent to the actual center, which takes about three days. Is there any possibility of the sample getting damaged and giving false negative results in this situation? Is this glans rash normal, as it has not healed for five months? Please help.

Answered by Dr. Rajesh Deshwal

Hi,

Welcome back to icliniq.com.

I understand your query.

You do not need any more tests. You do not need a PCR test as it is done to diagnose early infection. The sample is not damaged by transportation. Glans penis is normal.

Revert in case of queries.

Kind regards.

Patient's Query

Hi doctor,

Thanks for the reply.

Does it mean the PCR test will not give accurate results if done after three months? I am confused as some say PCR gives accurate results if done after 90 days. Is a PCR test report done after 90 days non-reliable? Or does it give a false negative report if done after 90 days?

Hi,

Welcome back to icliniq.com.

I understand your concern.

Kindly book a phone consultation to clear all your queries.

Kind regards.

Patient's Query

Hello doctor,

I did a viral load test exactly after six months. In the report, it is shown that the viral load is below 100 and in comments, it is written that the HIV RNA is not detected. What does this mean? I am attaching the report for your reference. Kindly interpret the results.

Answered by Dr. Rajesh Deshwal

Hello,

Welcome back to icliniq.com.

Glad to have you back.

From your report (attachments removed to protect the patient's identity), it means that you are negative for HIV. This test was not required anyways.

I wish you good health.

Thank you.

Patient's Query

Hi doctor,

Thank you for the reply.

I will do the Epstein-Barr virus test as well since my wife, my toddler, and I are all having neuropathy and swollen lymph nodes in the neck. Also, there is pain in the back muscles of both legs. Kindly advise.

Answered by Dr. Rajesh Deshwal

Hello,

Welcome back to icliniq.com.

Epstein-Barr virus test will not help you. Kindly stop self-diagnosis and treatment. Consult a local practitioner for your complaints and follow his advice for your symptoms.

I hope this helps.

Thank you.
