I am a 41-year-old male, I have itching on my penis and redness here and there. This is the second time I am taking the Utistat tablet but still keeps on itching. What could it be?

My diabetes is controlled under medication. I did have a fungal infection almost twice a year for the past ten years. I am sexually active.

This is balanitis (inflammation or redness of the glans penis is visible). The causes could be improper hygiene, scratching and rubbing, allergic reaction to irritant soap, infective cause (fungal infection most common), STI (sexually transmitted disease), uncontrolled diabetes, injury, and exposure to chemicals. I suggest you follow the following:

1. Practice good hygiene, and safe sex.

2. Avoid scented soaps and lotions.

3. Always clean the glans penis after retracting the foreskin.

4. Wash with warm water.

5. Neutral non-soap cleansers can be used like Cetaphil cleanser or no soap cleanser or Episoft cleanser to clean genitals with water.

6. Dry the area after cleaning.

7. Keep the area clean and dry.

8. Avoid moisture, as moisture favors fungal growth.

9. Apply Clotrimazole cream twice daily for two weeks.

10. Tablet Fluconazole (Microcrystalline cellulose and Dibasic Calcium Phosphate) 150 mg once a day after dinner for three days.

11. Tablet Teczine-M (Levocetirizine, Montelukast) once at night after food for seven days.

12. Maintain proper genital hygiene as hygiene plays a very important role to avoid recurrence.

13. Keep checking on sugar levels and maintain them in control.

14. Avoid scratching and rubbing of lesions.

15. Wear loose comfortable thin cotton inners.

16. Avoid friction or injury, and sex until the lesion heals.
