Patient's Query

Hi doctor,

I have been experiencing a wide variety of symptoms for the past year like fatigue, muscle and body aches, brain fog/forgetfulness, vertigo, headaches, heat intolerance, episodes of feeling shakey (blood sugar is normal), bowel changes (constipation), episodes of nausea, trouble sleeping, urge to urinate frequently mainly at night, anxiety (especially in morning), depression, lowered sex drive, tachycardia, episodes of bigeminy and trigeminy PVC, weight gain and inability to lose weight, exercise intolerance, episodes of sweating, stomach aches, and daily facial and neck flushing that last 4 to 6 hours at a time. My face and neck get so red and hot. It looks like having sunburn. I also face ringing in the ears with partial hearing loss for a few minutes, patchy purple areas on toes, and episodes where my hands and feet get really cold.

Here is all my diagnosed health conditions:

1. Inappropriate sinus tachycardia.

2. Left ventricle hypertrophy.

3. Mitral valve prolapse with slight regurgitation.

4. Crohn’s disease (mild case).

5. Spinal stenosis and degeneration.

6. Facet joint arthropathy.

7. History of DVT.

8. History of PE.

9. History of kidney stones.

I also have multiple lesions in my body and they do not know why. I have a anterior mediastinal lesion that is (0.5 X 0.7 X 1.8 inch) that apparently has been there for past 15 years. I have a lesion in both kidneys and also a subcutaneous one on my right thigh. I deal with rapid heart rates, and my electrophysiologist starts to wonder if an endocrine disorder causes it, considering my tachycardia episodes that happen either while I am sleeping or wake me up or happen the moment when I wake up in the morning. Thyroid panels have all been normal. Only thing abnormal is chronically elevated monocytes. Any advice or input you can provide would be greatly appreciated.

I am sorry to hear you are going through a lot. Your symptoms are varied and involve many systems. In terms of endocrine, you mentioned your thyroid function tests are fine. I would like to exclude pheochromocytoma and carcinoid syndrome along with any neuroendocrine tumor. Your symptoms might be due to these. So I would suggest you perform.

1) Serum metanephrine.

2) 5 Hydroxyindole acetic acid and chromogranin levels.

3) CT TAP (computed tomography of thorax, abdomen, and pelvis) if you have not done it previously.
