Hello doctor,
My daughter is 3 years old. Today I noticed a bump on her anus, normally she passes stool with excess pressure. So it seems to be like a hemorrhoid. I am attaching the picture. Kindly refer and advise us on her treatment and care to be taken. Please suggest medicines for the same as due to the COVID situation we cannot take her to the hospital. Please help.

What your daughter has is a skin tag at the anal verge which is an indicator of fissure-in-ano. A fissure is nothing but a wound over the anus. This is generally a very painful condition. This happens when babies pass hard stools and/or are constipated. So few things that you will need to do for your daughter are:
1. Sitz bath - make the baby sit in a tub of warm water for about 10 minutes, three times a day.
2. Apply ointment Anovate (Lignocaine steroid combination) after a sitz bath over the anal verge. This will reduce the pain over the anal region and she will be able to pass stools easily.
3. Give syrup Calpol 250 mg, one teaspoon three times a day for three days.
4. Syrup Lactulose two teaspoon in a glass of water, twice daily for one week followed by once daily for another one week.
5. Syrup Albendazole one teaspoon single dose tonight.
You should avoid giving her constipating diet for a month. Avoid chocolates, biscuits, and bakery products. She should eat lots of fruits and vegetables. She has to drink atleast 3.5 to 4 cups of water every day. The fissure might take about a month to go away completely but the symptoms will reduce in about five days if she follows all this strictly. The skin tag will take about three to six months to go away completely after the fissure heals.
