How to manage severe nasal congestion?

Hello doctor,

I am a 22-year-old woman and have severe nasal congestion, likely due to a cold or sinus infection (I have not seen a doctor yet). When Pseudoephedrine was not effective, the pharmacist suggested trying Xylometazoline and Ipratropium nasal sprays. Both provide some relief, but I forgot to ask if it is safe to use them together. Is it okay to take both at the same time until I can see my general practitioner?

It is fine to take Pseudoephedrine along with Xylometazoline and Ipratropium nasal sprays together. Both work to reduce nasal congestion and relieve a blocked nose. Pseudoephedrine tablets are absorbed into the bloodstream and act systemically, while Xylometazoline and Ipratropium nasal spray are applied directly to the nasal mucosa, providing local relief with minimal absorption into the bloodstream.

The two medications have slightly different mechanisms of action, targeting different receptors and signaling pathways. You only need to be cautious about their combined use if you have hypertension or elevated blood pressure. Otherwise, until you see your general practitioner, it is safe to use both. However, do not take Pseudoephedrine more than twice a day at normal strength. If using the maximum strength tablets, limit the dosage to once a day unless advised otherwise by your general practitioner.

In the meantime, follow these general guidelines until your general practitioner can provide a definitive diagnosis and recommend the most appropriate treatment for you.

  1. Drink plenty of hot water.
  2. After using Xylometazoline and Ipratropium nasal sprays, lie down for five minutes, then inhale steam. Repeat this three times a day. Do not use it continuously for more than four days, as it may cause rebound congestion.
  3. Avoid dust, cold foods, cold drinks, smoke, and cold weather.
  4. Avoid traveling and heavy exertion.
  5. Eat nutritious food rich in vitamins and minerals.
  6. After washing your hair, dry it promptly.
