Hello doctor,

I have high blood pressure and diabetes. Currently, I am taking Olmesartan medoxomil, Amlodipine, with Hydrochlorothiazide and Metoprolol succinate 25 mg for blood pressure, and Glimepiride with Metformin for diabetes. All my levels, including lipid profile, echocardiogram, HbA1c, etc., are normal. Two days ago, I suddenly experienced neck pain and blurred vision for a short period. After closing my eyes for a few minutes, I felt normal. However, the neck pain persisted, and later, I experienced a brief episode where I could not recollect people's names for five to 10 minutes, even though I was fully conscious. After about 10 minutes, my memory returned to normal. This occurred while I was under mild stress. I was diagnosed with spondylitis a long time ago, though I usually do not have any pain.

I would appreciate your guidance on what might have caused this and whether such symptoms are normal. Could it be related to my existing conditions? Please advise on precautions and any necessary diagnostic tests.

Since you have diabetes and hypertension, I would like to evaluate you for the possibility of a transient ischemic attack (TIA). I recommend undergoing an MRI (magnetic resonance imaging) of the brain and a color Doppler study. If the results are normal, the episode might have been due to postural fainting, cervical myalgia, or simply stress-induced.

In the meantime, I suggest incorporating regular exercises and practicing meditation to manage stress effectively. Never skip breakfast, avoid standing for prolonged periods, and drink plenty of fluids. Additionally, keep a close watch on your blood pressure levels.
