Hello doctor,

After my second child (born three years ago) I had PPH. It was a terrible experience and my ob/gyn literally had to pull the clots out with his arms while two nurses held me down. It had to be done so quickly. And now to this day, I always have pain before I get my period, and at random times it is so painful. Do you have any thoughts on what could be happening? He has requested a gynecological scan but when I book it, it needs to be within a few days of my period ending. I would really appreciate this as I am wondering if I am not really able to have any more kids? I am not sensitive and am happy to hear the truth.

Do not be stressed and think positive. It would be better if you send me your complete history of menses, gynecological, obstetrics, medicine, and medications you had ever. As far as I could understand, you had postpartum bleeding (after delivery of your second child). For that, clots were removed manually and it is a source of anxiety for you still. Postpartum hemorrhage (bleeding after delivery) could be mostly due to four 'T's: tissue, trauma, thrombin,and tone. For you, it was some retained tissue that caused bleeding and that removed even manually as well. I do not know your circumstances and situation of that time but it is past now. So, now think positive for future. Pain, before menses is part of the premenstrual syndrome or only pain, can be observed by normal females. I suggest having tablet Ponstan (Mefenamic acid) 500 mg eight hourly as per need. You can have from day two to five or total five days even. It will not only reduce flow but also pain. I do not have your complete history and reports. What I could understand, I am trying to help you.

  1. Have good nutritious diet with daily walk and exercise.
  2. Daily Folic acid 0.4 mg as you want to have a pregnancy.
  3. Have recreational activities and quit recreational drugs as these reduce fertility and can give anomalies to the baby.
  4. Have complete treatment of any health issue you have.
  5. Have sex in the fertile period of your cycles.
  6. Have early booking in pregnancy and stress on generalized health.

Same symptoms don't mean you have the same problem. Consult a doctor now!
