Hi doctor,

I am a 36-year-old male. I suffer from severe fatigue, unexplained weight gain, headache, sleeplessness, cognitive dysfunction, with reduced sex drive for the past three years. I have a blurry vision too. I have gone to many doctors. But, I am not feeling healthy. My testosterone ranges from 176 ng/dL to 325 ng/dL. My endocrinologist says that there is nothing to worry. My estradiol and prolactin levels are normal. My free T4 was 54 ng/ dl, and TSH was 1.69 mIU/L. My memory has become worse, and my head is fully pressurized. I had an MRI, and there was nothing seen on it. I need some help, and I do not know what to do. Currently, I am taking Wellbutrin XL.

I have gone through the query and the test results thoroughly (attachment removed to protect patient identity). You have depression. Having depression with moderate to severe degrees results in having all these symptoms. So, it would be helpful to know if these symptoms preceded the development of depression or came along with it. A headache and firm pressure could be a side effect of Wellbutrin XL (Bupropion). Fatigue, weight gain, memory impairment, lack of sleep, loss of libido, and suicidal intentions are all symptoms of severe depression. Treat your depression problem first.

You will see your symptoms reduce after taking adequate treatment for depression. I think you need to talk to your psychiatrist to readjust your dose and add other drugs. If these symptoms came way before the onset of depression, then they are unrelated to it. I suggest you check your adrenal hormone levels like serum cortisol. Please let me know whether these symptoms developed before or after the depression. Also, get the following investigations done. So that proper treatment can be initiated.
