I noticed a small bump on my penis this morning. It does itch or hurt. Please help.

After careful inspection of images (attachment removed to protect patient identity), I feel the first possible cause for small bumps is molluscum cantagiosum as there appears a small dent on the top of the lesion. The problem is often acquired through sexual contact and appears after months of exposure.

The second possible cause is chancre as lesions due to syphilis are also painless and, at the time of appearing, do not form any ulcer type. However, after a few days of existence, syphilitic lesions turn out as ulcers.

The third possibility for the lesion is allergic contact dermatitis.

To give a better diagnosis of lesions, I request you to provide the following details.

How long are these lesions present? Does it itch, pain, tingle, or cause any form of discomfort? Are you sexually active? If yes, have you ever had unprotected sex in the last six months? If yes, please mention the dates? Do you have multiple partners? Have you applied any new cream, lotion, or anything which was not part of your life until recently? Do you have any medical illnesses, such as allergies?

These lesions are present for two days. It do not itch, pain, tingle, or cause any form of discomfort. I am sexually active for ten years (with my wife only). Yes, I had unprotected sex in the last six months. No multiple partners. No new creams, or lotion, or anything. I only used a new soap. I do not have any medical illnesses, such as allergies.

After reviewing the provided details and images, I feel allergic dermatitis is most likely the cause of the bumps over the penis. Since the area affected involves the top of the penis and its skin called balanitis.

The treatment of balanitis depends on the cause, but since it seems allergic, we need to remove the new shower soap you started using recently. Instead, clean the area with simple lukewarm water or emollient.

In addition, personal hygiene, especially the perineal area, needs to be upgraded as this could contribute to the development of balanitis. For the treatment part, one percent of Hydrocortisone cream can be used for the resolution of symptoms.

Also, we need to look for any other symptoms as sometimes this could be an early manifestation of STDs and may require testing in a detailed manner.
