Hello doctor,
My father is 54 years old. His weight is 136 lbs, and his height is 5.5 feet. He was hospitalized last year because of lower back pain after sneezing. He was diagnosed to have lumbar strain versus HNP. He was again hospitalized three months back and was diagnosed with compression fracture L2. Recently he was again hospitalized and was diagnosed with severe anemia. He is suspected of having multiple myeloma. He has been positive for a monoclonal gammopathy test. He had experienced the CRAB symptoms. He has taken Decilone for the past three weeks, and we stopped the medication since he got a cough and instead gave him Poten-Cee as his supplement. He just takes Losartan and Purinase. Do we have to start chemotherapy now? And since he cannot sit or walk, can we just choose to use oral medications?

Monoclonal immunoglobulin has increased. So it seems to be multiple myeloma.
His creatinine and calcium are also increased. So treatment needs to be started.
I suggest you consult a hemato-oncologist for bone marrow aspiration and further management.
Treatment of multiple myeloma is not possible at home.
A bone marrow study needs to be done for further workup. Then appropriate chemotherapy can be started.

Hello doctor,
Thank you for your opinion doctor. We really want to bring him again to the hospital for the procedures like bone marrow aspiration and others. However, it is really hard for us to transport him since he is bedridden, and most hospitals near us do not accept patients as there are no available rooms to accommodate because of COVID-19 occupancies. In the meantime, can you suggest some possible management available for us at home to help him cope with the condition? Maybe some medications, supplements, foods allowed and not allowed, and some other routines might be of great help. Again, thank you so much for your time.

He needs to be put on chemotherapy once bone marrow aspiration is done.
Give him more green leafy vegetables, cooked oatmeal, fruits, yogurt, beans, protein foods, etc.
There is no particular recommendation for food intake in multiple myeloma.
