Hello doctor,

I am a 25-year-old male and I have never been able to fully retract my penis. I have severe phimosis. I know phimosis can be treated, but my biggest concern is that I now have blue-looking veins from the shaft of my penis to the actual penis head that is supposed to be exposed. Is it serious? I have an appointment with a urologist in two weeks but the suspense is really killing me. Is it common, or can it be treated? Will I be able to successfully retract the penis head? I also feel pain sensation when I touch the hole at the tip of the penis. Please help. Of note, I smoke marijuana. I have attached photos for your reference.

I have gone through the photograph (attachment removed to protect patient identity). It looks like a case of phimosis. It can be treated by a simple circumcision. Those veins seen are normally present. Do not worry about surgery. It will not affect the blood supply to your penis. Phimosis is a pretty common issue and as I said it just takes a minor surgery and a couple of weeks for recovery.

I have something like a frenulum attached to the penis head. I have two questions: Is it normal or something to be really concerned about? Can surgery fix this? I am sorry if I sound dumb but I am really nervous and scared about my condition. I have this "assume the worst mentality" in me and any reassurance will be greatly appreciated. I have seen the photograph (attachment removed to protect patient identity). That is normally present. It will be dealt with during circumcision. There is no need for so much anxiety. I suggest you continue your day to day activities normally and your urologist will take care of the rest.

Those veins that you see are not on the glans penis but the foreskin which will be removed during surgery. Your glans will be visible only after circumcision because of your phimosis. Do not worry that there will be no cosmetic or sexual adverse effects after a proper circumcision.

When I try to touch my glans through the little opening, it feels weird. It kind of pains a little bit. Please answer the following questions: Is that normal for glans that have never been exposed? With circumcision, will the pain or sensitivity eventually go away or be tolerable?

This is normal because your glans has never been exposed to the outside and hence it will be sensitive. This will persist for a few months at least after surgery after which the skin lining will get adapted to the exposure. But, this is a gradual process and takes time. However, it will eventually become normal again.

I have been looking up for circumcision and now I am freaking out a little bit. I read that a guy never lost the pain sensation on his glans after one to one and a half years of circumcision and his penis was never exposed like mine. I am nervous because I also feel pain whenever I touch my penis glans and if it is going to be fully exposed, probably never going to be able to wear underwear or pants or shorts ever again let alone work again. So my question is judging from the pictures that I showed you, is it possible to correct my problems without full circumcision? I am sure I still need surgery, but I hope a dorsal slit or something like that could be done for my problems.

Circumcision is most likely the only option available. Others do not give equal results. A dorsal slit is not very cosmetic. So far in my experience, I have never had a patient who has had persistent glans sensitivity. It usually disappears with time.

Do you know any good tips to help desensitize my glans once I get circumcised? It hurts when I touch the little opening, I imagine once circumcised that glans all over will be in pain keeping in mind that I have never retracted the head of the penis in 25 years. How long do you think I should be out of work?

It is a natural process. I suggest you apply some local anesthetic gel if the symptoms are troublesome, but the process itself involves a gradual change in the epithelial lining of the glans.
