Hi doctor,

I have had piles for a few years. Due to constipation, I have had severe pain and bleeding while passing stool. I have Smuth ointment, but it starts bleeding when I try to apply it. The anus is swelled up and too painful even to apply the medicine with fingers. Please suggest some medicine and remedies.

I read your query and understand your concern. Kindly share the medical records of hemorrhoid piles treatment. Hemorrhoids are swollen blood vessels in the lower portion of the large intestine (rectum). They are among the most common causes of anal ailments. Symptoms can range from mildly bothersome, such as pruritus, to quite concerning, such as rectal bleeding. Non-surgical medical management is the initial treatment of choice for grade I internal and non-thrombosed external hemorrhoids.

Hence, it would help if you took local warm baths (twice or thrice daily).

A high-fiber diet and external fiber intake such as Isapghula husk (stool bulking laxative) every night.

Adequate fluid intake At least 3 to 4 liters a day.

You should take proper anal hygiene and stool softeners.

Oral analgesics and topical anesthetic creams & ointments to relieve the pain.

Topical steroid cream (as per doctor's prescription).

Other medicines with repurposed usage.

1. Locally acting thrombolytic and fibrinolytic agents - Combination of two medicines: Tranexamic acid and Etamsylate. Etamsylate works by increasing the ability to stick together and form blood clots, whereas Tranexamic acid prevents the breakdown of the clots. These effects help to reduce the bleeding.

2. Calcium Dobesilate (microcrystalline cellulose) - Calcium dobesilate is a vasoprotective and venotonic agent. It is the calcium salt of dobesilic acid. It is a synthetic molecule with the ability to reduce capillary permeability in the body.

3. Surgical treatment - Surgical treatment is done for non-responsive patients with conservative medical management based on surgeons' assessments.
