Hi doctor,

I am 23 years old. Before moving to a new place, I was seeing my girlfriend every day. I had sex for some days. After that, I started to have pain in my penis with pimples. After reaching the new place, I started to do masturbation due to the urge of having sex. After that, I noticed some painless pimples with slight itching. I am assuming that it is not STD because my girlfriend got pregnant and she has tested for all. What is the reason behind it?

I have gone through the attached photo (attachment removed to protect patient identity). You seem to have genital herpes in your penis. You have mentioned that your girlfriend got tested for all STDs (sexually transmitted diseases). But genital herpes has a special feature. You can get an infection for months or years without showing symptoms. Suddenly, it can become symptomatic. It can happen one time or multiple times in the body. In the latter case, it is called reactivated herpes. Definitely, you need treatment. You can take Acyclovir for this condition. Make sure that you do not have any interactions with other medicines. I assume that you do not have plans to have intercourse or any other form of sexual activity. If so, then you need to use condoms as you are highly contagious in this condition. You should inform your girlfriend as well as the treating gynecologist to get advice to handle the specific situation.

You need to do a visual inspection by a dermatologist in person if there is no improvement.

Differential diagnosis

You can have a nest of warts and bullous eczema.

Probable diagnosis

You can have reactivated genital herpes.

Treatment plan

Preventive measures

Do not have unprotected sex until you get relief. Wash your hands when you touch your genital region. Wash your underpants above 60 degrees. Do not irritate your genital region and do not practice masturbation.
