Hi doctor,

I have had pimples for the last 20 days, I applied aloe vera plant and ice, but it does not work. How to remove the pimples and acne on my face?

I understand your concern.

Having gone through the history and image provided from your end (attachment removed to protect patient's identity), it is acne vulgaris. Acne is caused it multiple reasons, including genetic susceptibility, wherein there is a transmission from parents or ancestors genetically; due to climate change due to hormone fluctuations, especially in androgen levels. Sweating due to using skin care products with unusual ingredients, due to using medications for any underlying medical conditions, stress, due to dietary factors including intake of oily and fried food, milk and dairy products, red meat, sweets, etc.

Acne may be on and off, and each time they appear, it may be managed.

I suggest,

1. AHA glow S (glycolic acid, aloe vera, and citric acid) foaming face wash may be used twice daily in the morning and night. Apply a sufficient quantity on the face, massage, and gentle circular motions for one minute, and wash off. They may be continued to be used for months together.

2. Peroclin gel (Benzoyl peroxide and Clindamycin) 2.5 percent gel to be applied on the acne marks only in the morning after using the face wash. They are to be applied till the medicine absorbs into the lesions and should be kept without washing till the next wash.

3. Clinmiskin Ad gel (Adapalene and Clindamycin) is to be applied at night on alternate nights in the affected areas only. This is a strong medication and therefore is to be used with caution. A very small quantity of the medication may be applied in a very thin layer on the affected area.

4. Aziderm 10 percent (Azelaic acid) cream to be applied on the alternate nights alternating to the above gel (Clinmiskin Ad gel). This is also a very strong medication and is to be applied with caution. A very small quantity of the medication is to be applied to the affected areas only. In case of sensitive skin, this medication may be avoided for first-time users. It may result in irritation, burning sensation, redness, and itching sensation.

5. There are procedures, including glycolic acid chemical peels, that may be down in the face. They are done in sessions of at least six every two weeks and are done at the dermatologist clinic; therefore, I would like to advise you to undergo treatment from a dermatologist.

6. Capsule Doxycycline 100 mg twice daily after food for seven days. This is an antibiotic, and in case of any previous allergies to antibiotics, this medication may be avoided.

7. Tablet Pantop (Pantoprazole) 40 mg twice daily 30 minutes before breakfast and dinner to avoid acidity due to antibiotics to be taken for seven days.

8. Avoid oily and fried food, sweets, red meat, milk, and dairy products. Consume more fruits, vegetables, and fiber and at least ten glasses of water a day.

Acne occurs due to the above-mentioned external and internal triggers, and though external triggers may be controlled, internal triggers like genetic and hormonal factors may not be controlled; therefore, consistency with using medications is key and may have to be used for months to prevent recurrences.
