Hi doctor,

I am a 29-year-old female. Lifestyle is healthy with exercise and good food mostly. Since last year, on both sides of my cheeks especially on the right cheek, there are three to four pimples. They produce the white head dries off leaves a red scar and again another erupts. I have slightly oily skin and stops applying all the serums. Please help.

I have seen the pictures. (attachment removed to protect patient identity). These are Acne vulgaris (whiteheads and pustular). You can start Azelaic acid 20% cream in the morning for 20 days and Clindamycin 1% gel at night for 20 days and daytime non-oily/greasy sunscreen (Suncros aquagel) and one face wash (Ahaglow face wash). Also, take tablet Prednisolone 5 mg one tablet at night after food and 2.5 mg one tablet at the morning after food for 20 days, tablet Azithromycin 500 mg one tablet at the morning after food on Tuesday, Wednesday, and Thursday (three days in a week) for four weeks. The reason behind pimples or acne is mostly hormonal changes and oily skin. So these can subside with treatment but requires maintenance treatment even after the subsidence of pimples. Take a less glycemic diet and more protein diet. Review after 20 days. Note: This treatment is for non-pregnant woman and non-lactating mothers.

I am using Neutrogena face wash, should I switch? Is sunscreen important? Are all tablets necessary? Will this treatment help with the acne scars and the red patches that you can see in the pictures? Kindly assist further.

You can continue with your Neutrogena face wash. Sunscreen is important, apply at least at 8 AM and 12 PM (afternoon). There are two tablets and both are important. Take at least for 20 days. From your photographs (attachment removed to protect patient identity) you have more dark spots and few scars. For dark spots, I have given cream and sunscreen. Treatment of scar is different, it will come in picture after the subsidence of pimples.

Same symptoms don't mean you have the same problem. Consult a doctor now!
