Hi doctor,

I am having premature ejaculation, which I have had for a few years now. Normally I last for less than a minute. I was once prescribed Pari 10 mg, which did work to some extent but I discontinued it as it was making me feel drowsy and out of sorts. Can you recommend a fast-acting medicine which can be taken just before sex? A friend of mine told me to take Dapoxetine 60 mg, which I did one to two hours before sex and it really worked. Is it fine to continue the same as long as I take it like once or twice a week? I do not suffer from erectile dysfunction, but I hear that Dapoxetine with Cialis would really help my cause. So is it fine to take Dapoxetine 60 mg with Cialis, maybe couple of hours before sex? I will limit this to around once a week. I am 37 years old around 165 lbs and 5 feet 9' inches. I am generally in good health and fit. I do have cholesterol but do not take medicines, managing with exercise and diet.

I understand your problem. Premature ejaculation is a common condition among many males even though they do not really seek advice. First of all please note that there is no such medication proven to correct it 100 % or to act quickly. Non-Drug therapy including behavioral techniques helps to overcome it in 90 % of cases. One such method is stop-start technique. In which you or preferably your partner should stimulate your genital organ and when you feel like ejaculating, stimulation should be stopped for about 30 seconds or the till the passing feeling disappears. This should be done on regular basis before sex. Another method is the Squeeze Technique. It works the same way as the start and stop method. But, when you feel like you are reaching ejaculation, you or your partner squeeze the head of your penis until you lose the erection. Repeat this a few times before ejaculating. Some males find that if they think of something else during sex they can last longer.

Drugs including Dapoxetine blocks serotonin levels in the brain and known to be beneficial in some patients as you have already experienced. But these drugs have many troublesome side effects with long-term use like gastric disturbances, headaches, and poor sleep.

There are common modifiable reasons for this problem. Psychological stress, performance anxiety about sex, depression, and relationship problems with the partner can result in this. Therefore try to identify any underlying cause for that. May I know whether you have multiple partners or a single? If you have multiple do you experience the same problem with all partners?

Since you are otherwise healthy please be reassured that you do not have any significant problems with your sexual functions. This issue is fairly common in the society. If you are still experiencing after trying above behavioral techniques you may consult a consultant psychiatrist. But before that try to identify any psychological disturbances and try those behavioral techniques as drugs play a little role and cause unwanted side effects in the long run.

I do not intend to take the medicine daily. Just once a week. If so can I take Dapoxetine 60 mg along with Cialis? I do not intend to take this medicine on a daily basis. Just only once a week. I just need to know whether it is fine to take Dapoxetine 60 mg and Cialis together at the same time. I might take only once in two weeks.

Dapoxetine is given for premature ejaculationand Cialis (Tadalafil) is given for erectile dysfunction. As you do not have erectile dysfunction I do not see any rationale for combining it with Dapoxetine. Dapoxetine should ideally be taken daily in order to achieve full pharmacological benefit but which can result in those side effects.

However, rapid clearance of this drug from the body will lessen the side effects in comparison to other drugs of this group. Anyway, you may take these two drugs together if you wish as these two drugs were not known to interact together. But what I feel is that you need some psychological evaluation by a qualified person rather than taking over the counter medication. In many countries, these drugs are issued only for prescriptions by a qualified doctor.

It is preferably to be taken in the morning or daytime as it can cause poor sleeping. But you may take at any time as it is rapidly cleared from the body. Take after meals.
