Hello doctor,

Since two years, I am having pelvic pain, scrotal pain (scrotal veins) and the penis has become visibly smaller. There is erectile dysfunction, loose skin above the penis and between scrotum and anus, diarrhea, mood swings, muscle spasm, joints issues, GERD, hormonal changes (level of free testosterone is of a man aged 70, while I am 33), headaches and ringing in the ears.

What is the reason here for scrotal pain, pelvic tissues deformation, enlarged veins and gastric issues?

Do I have rectal cancer?

All your symptoms fit into changes concerned with the liver parenchymal disease with associated varices and enzyme deficiencies rather than rectal cancer. However, on observing your MRI (attachment removed to protect patient identity), I could deduce-

  1. There is no rectal or bowel wall thickening in the visualized bowel.
  2. No lymph nodes to deduce any metastatic disease.
  3. Right tunica vaginalis hydrocele with right varicocele is present.
  4. I would request you to get your liver function tests enzymes panel done and a routine abdomen ultrasound to evaluate liver echotexture. The preventive measure that you can take is prohibiting alcohol and other alcoholic beverages.

Just one more question about hydrocele in the scrotum. I understand it needs urological treatment, is that right? Can you determine whether there is an inguinal hernia on right side based on the MRI to provide my urologist more background information?

There was no hernia on either side in both the MRI (magnetic resonance imaging) that you had uploaded.
