Hello doctor,

I have a tooth on which root canal was performed around the end of last year. All had been fine since the procedure, but suddenly out of the blue, the tooth on which the root canal was performed became sensitive to hot water. Sensitivity is also felt when biting down on food. It started a few days ago. Apart from this irritation, no other symptoms or pain are felt.

It is possible that the root canal-treated tooth got reinfected. I suggest you start a three-day course of antibiotics and painkillers from today to prevent the root canal-treated tooth from getting reinfected or hypersensitive. Once a root canal is done, you cannot feel pain or sensitivity until the tooth or the gum gets infected again, which can happen in some people even after a few years. So I suggest you (consult with a specialist doctor, talk with him or her and take medicines with their consent) start first with Amox (Amoxicillin) 500 mg and painkiller Dolokind plus (Paracetamol and Aceclofenac ) twice daily. You can take one tablet after breakfast and one tablet after dinner. You can take Pan-D (Domperidone and Pantoprazole) before food if you have acidity issues. If you still feel persisting symptoms after the antibiotic and painkiller course prescribed, you will have to visit the dental surgeon in the clinic to get the tooth in question examined and the opposing tooth also. You may often find that the opposing tooth may be cavitated or decayed, causing sensitivity issues rather than the root canal-treated tooth itself. If you have any queries, please feel free to ask.

How can the reinfection occur when the pulp or the primary nerves have been removed?

That can happen because of various reasons. Sometimes due to root infection, stress, gum disease, or vitamin deficiencies. But the local cause should be treated, which is why antibiotic prophylaxis and lukewarm salt water rinse multiple times a day is effective. Rarely is it seen that incorrectly filled root canals or periapical infection, which can be caused due to the material used, can also cause reinfection. Still, I would advise that the dentist should check the opposing tooth.

I have decided not to go to the dentist at this point because the pain is annoying but nothing compared to when I first had the pulp infection six months ago. I have rinsed my mouth with Orasol, and now it is getting better. I will observe the condition for an additional three days.

You can take antibiotic prophylaxis if the pain gets severe. Usually, re-root canal treatment is just the last option when the infection is severe, leading to acute pain. Otherwise, it can be controlled by mouth rinsing and the antibiotic itself.
