Hello doctor,

I made this query in continuation with my previous one.

1. Do you have any ultrasound of scrotum reports and hormone assay reports?

I have attached the hormone assay report. In addition, I did a penile ultrasound test. The doctor said everything was good with me, and I could bring a report, but I never did the scrotum one.

2. Did you have a habit of excessive masturbation?

When I was at the age of 16 to 22, I used to do it daily. Then I figured that it was too much and needed to reduce, so I got to be able to do it one to three times a week till I got married at 31 years old.

Do you know a link to support me with perineal and pelvic exercises?

What are the best habits for walks to recover very fast?

How much to take of flex seeds?

What kind of dry fruits to take, and how many are the best portion for me?

How many weeks exactly to stop any sexual activity? Is it very hard to do it with my wife? I feel I suppose to have sex with my wife weekly, and the medicines support me to do it? Otherwise, I will be in big problems by ignoring sex.

I have had enough from all doctors, and I feel you may have the solution for me. However, I am always scared that the medicine will not work for me anymore. Can I use something daily to manage myself till I complete my treatment?

I reached a situation where I had no energy to handle the anger, trying a sexual activity, and to work out.

Your penile doppler report is completely normal, telling us that there is little to worry about. However, hormone assay is important if you have an erection problem, low desire, or premature ejaculation, so they must be done.

Generally, we need serum testosterone, prolactin, LH, and FSH to treat sexual dysfunction disorders in men. If you have had normal erections during masturbation since you started masturbation to the point of your marriage, I feel there is little to worry about. The most important thing during masturbation is not to apply excessive pressure as some men do during the height of excitement.

The best way to recover is to reduce stress, as stress is the most likely cause. Flax seeds are good for releasing sex hormones and optimizing your sexual health. Generally speaking, a tablespoon of flax seeds twice a day is recommended. For young men, a medium bowl (100 gram) of dry fruits is enough and help to rejuvenate vessels and muscles. Rather than the optimum size of each one, I feel a combination of all locally dry fruits will work better. I do not think the sexual activity needs to be stopped to recover sexual health.

Thank you for the reply.

I have been married for more than three years and never completed any sexual intercourse with my wife because of either erectile dysfunction or long or short orgasm duration of me. However, my wife never gets any orgasms at all. I examined myself many times with a urologist and did not have any medical problems; everyone told me it was a psychological problem. I used to take 5 mg of Tadalafil, and it never worked completely with me. Sometimes it works with 10 mg of Verdenafil. Sometimes it is worse than the Tadalafil. I need permanent treatment. I am still young, 34 years old, and I never know what is happening to me.

If you know of any medicines that can help and assure complete intercourse, please let me know about it.

Thank you for the reply.

1. I have attached the hormone profile, including serum testosterone, prolactin, LH, and FSH.

2. Yes, I often get erections at least five times a week in the morning when I wake up. It is rigid enough to have the ability of intercourse.

3. I cannot get an erection during masturbation unless I am watching porn which I stopped watching three years ago.

4. When I take medicine, if I'm able to insert my penis into my wife's vagina, then it either last very short, like in 10 to 20 seconds, I will cum, or it will last forever. Sometimes I cannot cum, which makes my wife feel weird about what happened.

5. Erectile dysfunction problem, when I get closer to my wife or walk or stand, I never can stay rigid.

6. I have attached the report of the serum lipid profile.

7. I have high LDL.

8. I am not taking any medications currently.

I have gone through all the attached reports (attachment removed to protect the patient's identity) and answers provided to my questions.

Per the available information, a PDE 5 inhibitor and Dapoxetine combination will work better for your sex needs. They generally contain 10 mg of Tadalafil and 30 mg of Dapoxetine to be taken before 90 minutes of intercourse. However, please consult a physician, discuss with them, and start taking the medications with their consent.

In addition, I feel there is a need for one-to-one talk as problems have a psychological component and must be understood without delay.
