Patient's Query

Hi doctor,

I am a two-time staph survivor who, after the second bout, was prescribed 500 mg twice a day of Dicloxacillin forever. As you can imagine, my gut has not been the same since and is not getting any better, even with daily Align probiotic.

Are there natural or herbal equivalents that could be considered? The health of my microbiome is at stake. I am almost 57 years old and otherwise consider myself to be pretty healthy.

Hi,

Welcome to icliniq.com.

I have gone through your history, and I understand that you are suffering from gut-related issues. Could you elaborate on what is disturbing you, whether it is persistent diarrhea or something else? And since when you have been on Dicloxacillin?

Patient's Query

Thank you doctor,

What is disturbing my gut consistency? My stool is never normal, sometimes close but usually more liquid, runny, and I have minor upset stomach often not always, but rumbling that leads to the loose stool.

I have been on Dicloxacillin since my second bout with staph, which was less than three years after my first staph immediately following my first aortic valve replacement (bovine).

For your (and my) benefit, I am attaching a detailed one-page medical history that includes all medications, timing of past surgeries and major events, and more.

Hi,

Welcome back to icliniq.com.

It is nice to hear back from you. I could see you have gone through a long medical journey. Your stool consistency issues could be mainly due to your gut microbiome disturbance, as you rightly pointed out as a consequence of long time antibiotic usage. Kindly take medicines as I have prescribed below regularly for two weeks and review it back with me. If not resolved, then we shall proceed for stool testing. And the medicines I am giving can be safely taken with your other regular medication.

Treatment plan

Tablet Rifaximin 400 mg three times daily after food for two weeks, Enterigermina respules (preprobiotic) three times daily after food for two weeks, tablet Rabeprazole 20 mg twice daily before food for two weeks.

Patient's Query

Thank you doctor,

What you are saying makes perfect sense. I assume at this point that I get your recommended prescription filled locally, but if that is not the case, please let me know. Do you think I can continue taking Dicloxacillin while combating it with what you are suggesting? My partner and I are more vegetarian, and my gut always feels better when I do not eat meat. Fish seems good, but red meat, in particular, does not do me any good.

Answered by Dr. Prathish Kumar

Hi,

Welcome back to icliniq.com.

You can very well get my medicines as a prescription locally. And regarding your query about Dicloxacillin, yes, that is an important step we need to decide. But you are considering your long medical history with repeated cardiac ailments. I would suggest you consult with your regular cardiologist regarding this and clarify whether you need to continue the antibiotics. If so, whether can you switch over to an alternative drug. That would be the right choice as just discussing over text about your cardiac illness and me suddenly changing your regular medicine is not correct.

You can discuss this with your cardiologist about our conversations. It would be good if that can be withheld for a few days, atleast for these two weeks. Your cardiologist will be the right person to decide upon it since he only knows your personal physical and medical status better than me.

And regarding meat, let us give your gut two weeks holiday period for meat as sometimes it may disturb our body's harmony with the medicines. Review back and update me after you have a word with your cardiologist.

Patient's Query

Thank you doctor,

I appreciate your responsiveness and details very much. I will heed your advice about the Dicloxacillin and consult my cardiologist about how I administer it. Can I order the medicines over-the-counter? I am happy to feel that your answers may help the struggles I have had for so long.

Answered by Dr. Prathish Kumar

Hi,

Welcome back to icliniq.com.

Yes, you can order them as per your convenience. Omeprazole can be taken instead of Rabeprazole, both belong to the same group, but Rabeprzole is little more potent than Omez. You can take either of them.

Patient's Query

Thank you doctor,

It has been a few weeks now and I am now on the recommended medicines. I do think things are a bit better. Do I need to continue with the Omeprazole longer than two weeks? I am doing very low dosage for my lower bowel, and now doing a pre- and pro-biotic. I will stay on this regimen, as my body seems to be handling it fine, too. Any other suggestions are always welcome.

Answered by Dr. Prathish Kumar

Hi,

Welcome back to icliniq.com.

If you feel your gut is getting better day by day, you can continue Omeprazole and pre probiotic on a daily basis until you feel you are well improved.

Let me tell you a personal experience. My wife suffered from a cholera pandemic. She developed post-infectious irritable bowel syndrome where she could not tolerate even a small amount of food, immediately she had to go restroom. She was afraid at the sight of food itself. So, we consulted a gastroenterologist and she was started on this regimen which she had to continue for nearly six months for her to recover completely. Now she is completely fine able to take nonveg and all kinds of food without any issue.

I am giving a similar regimen of hers to you as I myself has visualized the improvement after longterm regular medicine intake in my wife. So, need not worry, you can continue with this regimen on a long term also. It will not harm you in anyway even if take for long periods.

And regarding meat, if possible extend your holiday period for further two weeks and then start slowly in a step up fashion gradually increasing the quantities. But anytime if you feel consuming meat is disturbing the harmony, please do withhold it for few months. And yes before I forget, did you visit your cardiologist. What was his opinion and advice?

Patient's Query

Thank you doctor,

I appreciate your responsiveness very much and your personal experience that is related helps as well. As you have suggested, I will continue on the plan, as I do believe there has been and continues to be an improvement, albeit slowly. I suspect that I was bad enough where recovery, including full recovery, may take some time. I have not talked with my cardiologist yet about this, but I will reach out to him.

Answered by Dr. Prathish Kumar

Hello,

Welcome back to icliniq.com.

I have not given certain medicines my wife has taken among the regimen, but that was prescribed after an endoscopy and colonoscopic examination of her bowels.

And as you said, you are recovering, but in a slow pace. I would tell until it is on the improving trend that let us wait and continue the same regimen. Since your bowel had suffered for a long, I think it has to take its own time to recover from its hard times, although slowly but only to make you happy and serve you better for your taste buds in the future.

Same symptoms don't mean you have the same problem. Consult a doctor now!
