Hello doctor,

I have been diagnosed with lupus, I broke out in a full-body rash a few months ago and things went downhill from there. Currently, I have these blood rashes on my legs which sometimes show up on my arms. My legs become swollen with full of fluid. My blood pressure has been up and sometimes it shows 150/110 mmHg. I have a family history of blood clots and lymphomas. My blood test was seen to range in the normal high. My red blood cell count is always high and there is always some blood discharge in my urine. My lymph node on the left side of my neck was swollen last week, but has, been fine as far as now. Most doctor's seem to dismiss my symptoms but I feel something is brewing.

Lupus is an autoimmune vasculitis which is ten times more common in females in their second and third decade. Steroids are the common drugs to be used in its treatment. It will have a waxing and waning course most of the time. Red blood cells in urine could be because of renal involvement in lupus which we call lupus nephritis (a frequent complication in people who have systemic lupus erythematosus). Here the glomeruli are affected by the auto-antibodies. Your fatigue and high blood pressure are most probably related to lupus. Thrombus formation is also very common in lupus, especially deep vein thrombosis (DVT) (a medical condition that occurs when a blood clot forms in a deep vein) of legs. As you mentioned your red blood cells are always high and you experience fatigue, I would suggest you get a comprehensive blood examination and look for packed cell volume to rule out polycythemia. This condition usually presents with itching, especially after hot showers. As mentioned earlier lupus will have more waxing and waning course and may involve any body organs with regular follow-up with your ​doctor is very important. I strongly suggest you rule out polycythemia (an abnormally high number of red blood cells in the blood, as a primary disease or secondary condition) in your case.
