Hi doctor,

I am a 28-year-old female. My height is 5 feet and 10 inches. I have been suffering from tension headaches for weeks. It stops now, but it has been on and off for months. I received stronger painkillers as well as a stronger muscle relaxer. The last painkiller I had earlier has not helped, so I had additional painkillers and still suffer. I want to know when I should go to the emergency room to look into further. I cannot function like this and cannot look after my two young children. I am also extremely tired every day. I have seen my general physician six times in the past few months about this, and it is always classed as a tension headache. Each time a stronger painkiller and muscle relaxer is given. It does not help.

So do I go back and get the same treatment, or do I go to an emergency room?

Is it a waste to speak to the emergency room doctors and nurses?

I would like to ask few details about your headaches :

1. Since when you started to have these headaches?

2. Does this severe headache occur in episodes, too? Or some persistent headache only?

3. If episodic, do you also feel nausea or vomiting during headaches?

4. If episodic, one episode of a severe headache last for how long? (without painkiller).

5. How are your sleep and mood in the last two to three weeks?

6. Have you ever undergone a magnetic resonance imaging (MRI) or computed tomography scan (CT scan) of your head for these headaches?

The investigations to be done are as follows.

  1. Magnetic resonance imaging (MRI) brain.
  2. Ophthalmic examination to see intraocular pressure and fundus
  3. Thyroid function test.

Differential diagnoses are tension-type headaches and migraine headaches.

Please reply to my questions with a report of tests, then need to start some medication to be taken regularly.
